Mycophenolate 1500 mg BID is the Immediate-Release (IR) Formulation
A dose of 1500 mg twice daily refers to mycophenolate mofetil (MMF) immediate-release formulation (CellCept), not the extended-release formulation. The extended-release formulation uses mycophenolic acid (enteric-coated mycophenolate sodium) and is dosed differently at 720-1080 mg twice daily 1.
Formulation-Specific Dosing
Immediate-Release (IR) - Mycophenolate Mofetil
- Standard therapeutic dose: 1000-1500 mg twice daily 1
- Starting dose: Typically 500 mg twice daily, gradually increased 1
- Available as: CellCept capsules (250 mg), tablets (500 mg), or oral suspension 2
- FDA-approved dosing for transplant:
Extended-Release (ER) - Mycophenolic Acid
- Therapeutic dose: 720-1080 mg twice daily 1, 3
- Conversion: 360 mg mycophenolic acid ≈ 500 mg mycophenolate mofetil 4
- Used when: GI side effects occur with IR formulation 3
Clinical Context
The 1500 mg twice daily dose is commonly used in:
- Autoimmune conditions (off-label): Systemic lupus erythematosus, pemphigus vulgaris, interstitial lung disease 1, 3
- Transplant rejection (higher end of dosing): Cardiac and hepatic transplantation 2
- Refractory cases: When standard 1000 mg twice daily is insufficient 3
Important Caveats
The formulations are NOT interchangeable on a milligram-per-milligram basis. If you see 1500 mg twice daily written, this is mycophenolate mofetil IR. The enteric-coated formulation would be prescribed as 720-1080 mg twice daily of mycophenolic acid 5.
Common prescribing error: Confusing the two formulations can lead to under- or over-dosing. Always verify which specific product (CellCept vs. Myfortic/enteric-coated) is intended when doses seem unusually high or low.
Monitoring requirements remain the same regardless of formulation: CBC with differential and comprehensive metabolic panel at baseline, 2-3 weeks after starting or dose changes, then every 3 months on stable dosing 1.
