Lung Cancer Treatment Pathway
A comprehensive lung cancer treatment pathway should be structured around molecular testing, histology, performance status, and stage, with treatment decisions made through multidisciplinary tumor boards as recommended by both ESMO and NCCN guidelines. 1, 2
Initial Diagnostic Workup and Staging
Essential Baseline Assessment
- Complete history: smoking status, weight loss, comorbidities, performance status (PS) using ECOG scale
- Physical examination with focus on respiratory symptoms, neurological signs
- Laboratory: complete blood count, renal/hepatic function, bone biochemistry (CEA not routinely recommended) 2
Imaging Requirements
- Contrast-enhanced CT chest and upper abdomen (mandatory)
- PET-CT for mediastinal lymph node and distant metastasis assessment (highest sensitivity) 2
- Brain MRI (more sensitive than CT) for:
- All patients eligible for locoregional treatment
- Patients with neurological symptoms/signs
- Consider for all metastatic patients given high CNS metastasis rates 2
- Bone scan only if clinical suspicion and lesions not evaluable on CT 2
Pathological Confirmation
- For abnormal mediastinal/hilar nodes: EBUS/EUS preferred over surgical staging 3
- Mediastinoscopy if EBUS/EUS negative but high clinical suspicion 3
- Confirm solitary metastatic sites cytologically/histologically before declaring stage IV 2
Molecular Testing Strategy
Mandatory Testing for All Non-Squamous NSCLC and Selected Squamous Cases
Test immediately at diagnosis, do not wait for treatment failure:
- EGFR mutations (exon 19 deletions, exon 21 L858R, T790M, uncommon mutations including S768I, L861Q, G719X) 1
- ALK rearrangements (FISH standard, IHC for screening) 1
- ROS1 rearrangements 1
- BRAF V600 mutations 1
- PD-L1 expression (for immunotherapy selection) 1
- Consider: RET, MET exon 14 skipping, MET amplification, HER2, NTRK fusions 1
Treatment Pathways by Molecular Profile
PATHWAY 1: EGFR-Mutated NSCLC (Exon 19 Del or L858R)
First-Line Treatment
Preferred option: Osimertinib [I, A] - superior PFS, CNS activity 1
Alternative options (all Level I, A):
- Erlotinib, gefitinib, or afatinib (no consensus on preferred choice among these three) 1
- Dacomitinib (when approved) 1
- Erlotinib + bevacizumab [II, B] 1
- Gefitinib + carboplatin + pemetrexed [I, B] (not EMA-approved) 1
Key principle: All patients receive EGFR TKI regardless of PS, gender, smoking history, or histology 1
Progression on First-Line EGFR TKI
Step 1: Test for T790M mutation [I, A]
- Liquid biopsy first (if negative, tissue re-biopsy if feasible) 1
- Rule out small cell transformation (occurs ~5% of cases) 4
Step 2: Treatment based on T790M status
If T790M positive:
- Osimertinib (if not used first-line) [I, A] - standard of care 1
If T790M negative:
- Platinum-based doublet chemotherapy [I, A] 1
- Category 1 preferred: Amivantamab + carboplatin + pemetrexed (for non-squamous with multiple lesions) 4
Step 3: Oligoprogression management
- Continue EGFR TKI + definitive local therapy (SABR/surgery/SRS) for limited progression sites [III, B] 1, 4
- For CNS progression: SRS ± surgical resection for symptomatic lesions; SRS for asymptomatic lesions at risk 4
EGFR Uncommon Mutations (S768I, L861Q, G719X)
- First-line: Afatinib or osimertinib [I, A] 4
- Response rates: S768I (100%), G719X (77.8%), L861Q (56.3%) with afatinib 4
PATHWAY 2: ALK-Rearranged NSCLC
First-Line Treatment
Preferred options (superior to crizotinib):
- Alectinib [I, A] - longer PFS, lower toxicity, excellent CNS activity 1
- Brigatinib [I, B] - longer PFS, CNS activity (not EMA-approved) 1
Alternative options:
For CNS involvement at diagnosis: Alectinib [III, A], brigatinib [III, B], or ceritinib [IV, B] preferred over crizotinib 1
Second-Line After ALK TKI Progression
If crizotinib used first-line:
- Alectinib or ceritinib [I, A] 1
If second-generation ALK TKI used first-line:
- Next-generation ALK inhibitors (brigatinib, lorlatinib) [III, B] - not EMA-approved 1
If no prior crizotinib:
- Crizotinib as next-line [I, A] 1
Oligoprogression: Continue ALK TKI + local therapy [III, B] 1
PATHWAY 3: ROS1-Rearranged NSCLC
- First-line: Crizotinib [III, A] 1
- Second-line (if crizotinib-naive): Crizotinib [III, A] 1
- After crizotinib: Platinum-based chemotherapy [IV, A] 1
- Ceritinib (crizotinib-naive, not EMA-approved) [III, C] 1
PATHWAY 4: BRAF V600-Mutated NSCLC
- First or second-line: Dabrafenib + trametinib [III, A] 1
- After BRAF/MEK inhibition: Platinum-based chemotherapy [IV, A] 1
PATHWAY 5: Other Actionable Drivers
Currently investigational - enroll in clinical trials [III, C]:
- RET fusions
- MET amplification
- MET exon 14 skipping variants (crizotinib shows potential) 1
- HER2 dysregulation
- NTRK fusions 1
Treatment Pathways for Non-Driver Mutation NSCLC
PATHWAY 6: Non-Squamous NSCLC Without Actionable Mutations
First-Line Treatment by PD-L1 Status
PD-L1 ≥50% (PS 0-2):
- Preferred: Pembrolizumab monotherapy [I, A] 1
PD-L1 1-49% or PD-L1 <1% (PS 0-1):
- Preferred: Platinum-doublet chemotherapy + immunotherapy
Duration: 4 cycles recommended, maximum 6 cycles [II, B] 2
PS 2 Patients
- Single-agent chemotherapy (gemcitabine, vinorelbine, taxanes) [I, B] 2
- Platinum-based combinations may be considered [II, B] 2
PS 3-4 Patients
Elderly Patients (≥70 years)
PS 0-1 with adequate organ function:
Unfit or comorbid elderly:
- Single-agent chemotherapy [I, B] 2
Second-Line Treatment (PD-L1 Naive)
Preferred (irrespective of PD-L1):
- Nivolumab [I, A] (squamous and non-squamous) 1
- Pembrolizumab (if PD-L1 >1%) [I, A] 1
- Atezolizumab [I, A] 1
If immunotherapy unsuitable:
- Pemetrexed (non-squamous only) [I, B] - better tolerability 1
- Docetaxel [I, B] 1
- Nintedanib + docetaxel (adenocarcinoma) [II, B] 1
- Ramucirumab + docetaxel (PS 0-2) [I, B] 1
After Progression on First-Line Pembrolizumab
- Platinum-based chemotherapy [V, B] 1
PATHWAY 7: Squamous Cell NSCLC Without Actionable Mutations
First-Line Treatment
PS 0-2:
- Platinum-based doublet chemotherapy [I, A] 2
- Consider immunotherapy combinations based on PD-L1 status (similar to non-squamous)
PS 2:
- Single-agent chemotherapy [I, B] 2
Second-Line Treatment
Preferred:
If immunotherapy unsuitable:
- Docetaxel [I, B] 1
- Ramucirumab + docetaxel [I, B] 1
- Afatinib (PS 0-2, unfit for chemotherapy/immunotherapy, EGFR wild-type/unknown) [I, C] 1
Special Clinical Scenarios
Oligometastatic Disease (3-5 Metastases)
After initiating systemic therapy:
- Definitive local consolidative therapy (SABR, surgery, or IGTA) [III, B] 4
- Particularly important for EGFR/ALK-positive patients on TKI therapy 4
Brain Metastases
Symptomatic lesions:
Asymptomatic lesions at risk:
WBRT considerations:
- Selected patients with favorable prognostic factors [III, C]
- Avoid in RPA class III (dismal prognosis) - best supportive care only [I, A] 1
Other Palliative Radiotherapy Indications
- Hemoptysis, symptomatic airway obstruction [III, B] 1
- Painful chest wall/bone metastases [II, B] 1
- Superior vena cava syndrome [II, B] 1
- Spinal cord compression (early RT) [II, B] 1
Key Clinical Pitfalls to Avoid
- Do not start chemotherapy before molecular testing results in non-squamous NSCLC - delays appropriate targeted therapy
- Do not use pemetrexed in squamous histology - ineffective and potentially harmful 2
- Do not stop EGFR/ALK TKI at first radiological progression if patient has clinical benefit - consider oligoprogression and local therapy 1
- Do not use immunotherapy as preferred option in EGFR-mutated NSCLC after TKI failure - less effective 4
- Do not forget T790M testing after EGFR TKI progression - osimertinib highly effective for T790M-positive disease 1
- Do not use bevacizumab in squamous histology - contraindicated due to bleeding risk 2
- Do not offer aggressive treatment to PS 3-4 patients without actionable mutations - best supportive care improves quality of life 2
Treatment Decision Algorithm Summary
Step 1: Confirm histology (squamous vs non-squamous) and stage
Step 2: Assess performance status (ECOG 0-4)
Step 3: Complete molecular testing (mandatory for non-squamous, selected squamous)
Step 4: Treatment selection:
- If actionable mutation present → Targeted therapy (see specific pathways above)
- If no actionable mutation → Chemotherapy ± immunotherapy based on PD-L1 and histology
- If PS 3-4 without actionable mutation → Best supportive care
Step 5: At progression → Re-biopsy when feasible, assess for resistance mechanisms, consider local therapy for oligoprogression
Step 6: Subsequent lines guided by prior therapies, molecular profile, and performance status
