Chest mein kharas (crepitus) ka matlab subcutaneous emphysema ho sakta hai, jo ek medical emergency hai aur turant evaluation ki zaroorat hai

Agar aapko chest mein crackling/crepitus feel ho rahi hai jo skin ko touch karne par mehsoos hoti hai, toh yeh subcutaneous emphysema ho sakti hai - yeh condition turant medical attention maangti hai kyunki yeh pneumothorax, pneumomediastinum, ya chest trauma se associated ho sakti hai 1, 2.

Kharas ke mukhya causes:

1. Subcutaneous Emphysema (Skin ke neeche hawa)

  • Yeh tab hota hai jab hawa chest cavity se leak hokar skin ke neeche aa jati hai
  • Pehchaan: Skin ko dabane par "bubble wrap" jaisa crackling feel hota hai, neck aur shoulders mein swelling ho sakti hai 1
  • Common causes:
    • Pneumothorax (lung collapse)
    • Pneumomediastinum (chest ke beech mein hawa)
    • Chest trauma
    • Zor se khaansne, ulti karne, ya heavy exercise ke baad 1
    • Chest tube drainage ke complications 2

2. Lung Crackles (Phephdon ke andar ki awaaz)

  • Yeh stethoscope se sunayi deti hai, patient ko directly feel nahi hoti
  • Associated conditions:
    • Pneumonia: Bukhar (>38°C), saans lene mein takleef, chest pain ke saath 3
    • Heart failure
    • Interstitial lung disease (fibrosis) 4
    • Crackles airways ke suddenly khulne/band hone se banti hain 5, 6

3. Noisy Pneumothorax

  • Chhoti left-sided pneumothorax clicking ya crackling sound produce kar sakti hai, especially heart ke paas 7
  • Yeh pericarditis jaisa lag sakta hai

Aapko turant doctor ke paas jana chahiye agar:

  • Emergency signs:
    • Saans lene mein mushkil
    • Chest pain
    • Neck ya shoulders mein swelling
    • Skin ko touch karne par crackling feel ho
    • Bukhar >38°C
    • Tez saans (tachypnea)

Evaluation kaise hoga:

  1. Physical examination: Doctor skin ko touch karke dekhega ki subcutaneous emphysema hai ya nahi
  2. Chest X-ray: Yeh zaruri hai pneumothorax, pneumomediastinum, ya pneumonia ko rule out karne ke liye 3, 1
  3. CT scan: Agar X-ray mein kuch clear nahi dikha toh CT scan ki zaroorat pad sakti hai 1

Management:

Agar subcutaneous emphysema hai:

  • Conservative treatment: Zyada tar cases mein yeh apne aap theek ho jati hai, sirf observation chahiye 1
  • Hospital admission: 2-3 din ke liye monitoring zaruri hai
  • Underlying cause ka treatment: Pneumothorax ko chest tube se treat karna pad sakta hai 2

Agar pneumonia hai:

  • Antibiotics local guidelines ke according 3
  • CRP level >30 mg/L pneumonia ki taraf point karta hai 3

Important caveats:

  • Subcutaneous emphysema prolonged hospital stay aur increased mortality se associated hai, isliye serious lena chahiye 2
  • Young, healthy adults mein forceful vomiting, coughing, ya strenuous exercise ke baad spontaneous pneumomediastinum ho sakti hai 1
  • Agar neurological symptoms hain (bahut rare), toh surgical intervention ki zaroorat pad sakti hai 1

Bottom line: Chest mein crepitus feel hona ek warning sign hai jo immediate medical evaluation maangta hai - yeh self-limiting condition ho sakti hai ya serious underlying problem ka indication. Delay mat kijiye.
