Grade 2 Fatty Liver ka OPD Treatment Protocol

Grade 2 fatty liver (NAFLD) ke liye sabse important treatment lifestyle modification hai - specifically 5-7% weight loss target karna, diet mein 500 kcal/day ki kami karna, aur moderate intensity exercise 30 minutes se zyada, hafte mein 3 baar se zyada karna chahiye. 1

Initial Assessment aur Risk Stratification

Patient ko pehle FIB-4 score calculate karna chahiye fibrosis ka risk assess karne ke liye 2:

  • Low risk (FIB-4 <1.3): Primary care mein manage kar sakte hain
  • Indeterminate (FIB-4 1.3-2.67): Transient elastography (FibroScan) ya ELF test consider karein
  • High risk (FIB-4 >2.67): Hepatologist ko refer karein

Metabolic comorbidities check karein: diabetes, hypertension, dyslipidemia, obesity.

Lifestyle Modification Protocol (Sabhi Patients ke liye Mandatory)

Weight Loss Target 1

  • 5-7% weight loss: Liver fat aur inflammation kam hota hai
  • 7-10% weight loss: Fibrosis improve hota hai
  • Progressive weight loss: Maximum 0.5-1 kg/week (rapid weight loss se fibrosis badh sakta hai)

Diet Plan 1, 2

Daily calorie intake:

  • Mard: 1500-1800 kcal/day
  • Aurat: 1200-1500 kcal/day
  • Total energy intake mein 500 kcal/day ki kami

Mediterranean diet pattern recommend karein 2:

  • Low carbohydrate aur saturated fat
  • Fructose-enriched soft drinks avoid karein
  • Fiber aur antioxidant-rich fruits/vegetables increase karein
  • Monounsaturated fats prefer karein

Exercise Protocol 1

Minimum requirement:

  • Moderate-intensity exercise: 30 minutes se zyada, hafte mein 3 baar se zyada
  • Moderate-intensity activities (3-6 METs): Brisk walking, slow cycling, swimming, dancing
  • Vigorous-intensity activities (>6 METs): Running, fast cycling, aerobics, competitive sports

Important: Aerobic aur resistance exercise dono equally effective hain. Patient ki preference aur cardiopulmonary fitness ke according choose karein 1.

Alcohol Restriction 1

  • Heavy alcohol consumption (mard ≥210 g/week, aurat ≥140 g/week) strictly avoid karein
  • Light-moderate drinking bhi caution ke saath - preferably avoid karein

Pharmacological Management

Agar Diabetes hai (Type 2 DM + NAFLD) 2

First-line options:

  1. GLP-1 Receptor Agonists (Liraglutide, Semaglutide): Steatohepatitis improve karta hai aur cardiovascular benefit bhi hai 2
  2. Pioglitazone (30-45 mg/day): Histologic improvement proven hai, lekin fibrosis improvement variable hai 1, 2, 1
  3. SGLT2 Inhibitors: Cardiovascular benefit aur weight loss ke liye, lekin direct liver benefit ka evidence limited hai 2

Avoid for liver treatment:

  • Metformin: Glucose control ke liye continue kar sakte hain, lekin steatohepatitis improve nahi karta 2

Agar Diabetes nahi hai (Non-diabetic NASH) 2, 1

Vitamin E (800 IU/day): Consider kar sakte hain, lekin advanced fibrosis ya diabetes mein evidence limited hai 2.

Common Pitfall: Pioglitazone se weight gain ho sakta hai (2-3 kg), isliye patient ko counsel karna zaroori hai 1.

Comorbidities ka Management 3, 4

Dyslipidemia

  • Statins: NAFLD mein safe aur recommended hain, hepatotoxicity ka risk minimal hai
  • QRISK3 >10% ya diabetes hai toh statin start karein 3

Hypertension

  • Standard guidelines ke according manage karein
  • Cardiovascular risk reduction priority hai 3

Follow-up Protocol

Regular monitoring:

  • Liver function tests (ALT, AST) har 3-6 mahine
  • Weight, BMI, waist circumference track karein
  • Diabetes/lipid profile monitoring
  • FIB-4 score yearly reassess karein 2

Referral to Hepatologist agar: 2

  • FIB-4 >2.67 ya LSM >12 kPa
  • Persistently elevated transaminases despite lifestyle modification
  • Clinical signs of advanced liver disease (ascites, encephalopathy, varices)

OPD Prescription Format

Rx:
1. Lifestyle Modification:
   - Weight loss target: 5-10% (0.5-1 kg/week)
   - Diet: Mediterranean pattern, 500 kcal/day reduction
   - Exercise: Brisk walking 30-45 min, 5 days/week
   - Alcohol: Complete avoidance

2. Agar Diabetes hai:
Tab. Pioglitazone 30 mg OD (ya GLP-1 RA)

3. Agar Dyslipidemia hai:
   Tab. Atorvastatin 10-20 mg HS

4. Investigations:
   - LFT, Lipid profile - 3 months baad
   - FIB-4 score - yearly
   - USG abdomen - 6 months baad

5. Follow-up: 3 months

Critical Point: Grade 2 fatty liver mein pharmacotherapy optional hai unless diabetes ya biopsy-proven NASH hai. Lifestyle modification hi cornerstone of treatment hai 1, 5.
