Ascites ka OPD Treatment

Cirrhotic ascites ke liye first-line treatment mein sodium restriction (2 gram/din se kam) aur combination diuretic therapy - Spironolactone 100 mg + Furosemide 40 mg daily - dena chahiye, jo har 3-5 din mein dose badhaya ja sakta hai (maximum Spironolactone 400 mg aur Furosemide 160 mg tak). 1, 2

OPD Prescription ka Algorithm

Pehli Visit par:

Dietary Advice:

  • Namak intake 5 gram/din se kam (2 gram sodium = 88 mmol/din) 1, 3
  • Khane mein upar se namak na dalein
  • Packaged/ready-made khana avoid karein
  • Protein intake badhayein: 1.2-1.5 g/kg/din 3
  • Pani restrict karne ki zaroorat nahi jab tak serum sodium >125 mmol/L hai 1, 2

Diuretic Prescription:

Naye ascites ke case mein (pehli baar):

  • Tab. Spironolactone 100 mg - subah ek baar 1, 3, 2
  • Agar response slow ho ya patient hospitalized hai, to shuru se hi combination dein

Purane/recurrent ascites mein:

  • Tab. Spironolactone 100 mg - subah ek baar
  • Tab. Furosemide 40 mg - subah ek baar (saath mein) 1, 4, 2
  • Yeh combination fast response deta hai aur potassium normal rakhta hai

Follow-up Strategy (har 3-5 din):

Monitor karein:

  • Weight daily (target: 0.5 kg/din weight loss agar edema nahi hai) 3
  • Serum electrolytes (sodium, potassium)
  • Serum creatinine
  • Spot urine sodium/potassium ratio (agar >1 hai to patient respond kar raha hai) 2

Dose Titration:

  • Agar weight loss inadequate hai, to dono drugs ka dose saath mein badhayein
  • 100:40 ratio maintain karein (Spironolactone:Furosemide)
  • Har 3-5 din mein dose adjust karein (Spironolactone ki long half-life ki wajah se) 1, 4, 2
  • Maximum dose: Spironolactone 400 mg + Furosemide 160 mg 1, 3

Common Complications aur Management

Hyperkalemia (K >6 mmol/L):

  • Spironolactone kam karein ya band karein 3

Hypokalemia (K <3 mmol/L):

  • Furosemide kam karein ya temporarily band karein 3, 4

Severe Hyponatremia (Na <125 mmol/L):

  • Diuretics band karein
  • Fluid restriction 1-1.5 L/din 1, 2

Acute Kidney Injury (Creatinine >0.3 mg/dL increase in 48 hours):

  • Diuretics temporarily band karein
  • Volume expansion with normal saline 1, 3

Hepatic Encephalopathy:

  • Diuretics reduce karein ya band karein 3

Gynecomastia (Spironolactone se):

  • Amiloride 10-40 mg/din switch kar sakte hain (lekin kam effective hai) 4, 2

Tense Ascites ke liye

Agar patient ko breathing difficulty ya severe abdominal distension hai:

  • Large volume paracentesis (LVP) pehle karein
  • 5 L se zyada fluid nikalne par: IV Albumin 6-8 gram per liter of ascites removed 1, 3, 5
  • Paracentesis ke baad diuretics shuru karein maintenance ke liye 6

Refractory Ascites ki Pehchan

Agar maximum dose diuretics (Spironolactone 400 mg + Furosemide 160 mg) par bhi 1 hafte tak response nahi aata aur sodium restriction follow kar rahe hain, to refractory ascites consider karein 3, 6, 3. Aise patients ko:

  • Repeat LVP har 2 hafte
  • TIPS evaluation
  • Liver transplant assessment 6, 2

Important Caveats

  • NSAIDs bilkul avoid karein - yeh diuretic response ko kharab kar dete hain 6
  • Bed rest mandatory nahi hai, lekin excessive activity avoid karein 4, 6
  • Dietician consultation strongly recommended hai compliance ke liye 1, 2
  • Agar alcohol-related cirrhosis hai to complete abstinence zaroori hai 6
  • Har patient ko liver transplant evaluation consider karein 6, 2
