Panitumumab is NOT Compatible with D5W
Panitumumab (Vectibix) must be diluted only with 0.9% sodium chloride injection (normal saline), not with 5% dextrose in water (D5W). 1
Preparation Requirements
The FDA-approved prescribing information explicitly states the following preparation protocol 1:
- Dilute panitumumab to a total volume of 100 mL with 0.9% sodium chloride injection, USP
- For doses exceeding 1000 mg, dilute to 150 mL with 0.9% sodium chloride injection, USP
- Do not exceed a final concentration of 10 mg/mL
- Do not mix panitumumab with, or administer as an infusion with, other medicinal products
- Flush line before and after administration with 0.9% sodium chloride injection, USP
Why This Matters
The drug label provides no mention of D5W as an acceptable diluent, and explicitly restricts dilution to normal saline only. This is a critical safety specification that must be followed 1.
Administration Details
- Infuse doses ≤1000 mg over 60 minutes (can reduce to 30-60 minutes for subsequent infusions if first dose tolerated)
- Infuse doses >1000 mg over 90 minutes
- Use a low-protein-binding 0.2 µm or 0.22 µm in-line filter
- Administer via infusion pump through peripheral IV line or indwelling catheter 1
Storage of Diluted Solution
Once diluted in 0.9% sodium chloride:
- Use within 6 hours if stored at room temperature
- Use within 24 hours if stored at 2-8°C (36-46°F)
- Do not freeze 1
Clinical Context
While D5W is commonly used for other monoclonal antibodies and medications, panitumumab's formulation (containing sodium acetate and sodium chloride in its base formulation) 1 requires normal saline as the only approved diluent. Using D5W could potentially affect drug stability, protein aggregation, or therapeutic efficacy, though the manufacturer has not provided compatibility data with D5W.
