From the Research
For blepharitis associated with arthritis, patients should typically use erythromycin eye ointment for 90 days as an initial treatment course, as evidenced by the most recent study 2. Apply a small amount (about 1 cm) to the eyelid margins 2-4 times daily. After this initial period, reassess the condition. If symptoms have improved but not fully resolved, continue for an additional week. For chronic cases, your doctor may recommend a maintenance regimen of applying the ointment 1-2 times daily for several weeks to months, often just at bedtime. It's crucial to combine this treatment with daily eyelid hygiene, including warm compresses and gentle cleansing of the eyelids. This helps remove debris and reduces bacterial load, enhancing the effectiveness of the antibiotic. Erythromycin works by inhibiting bacterial protein synthesis, thereby reducing the infection driving the inflammation. However, long-term use should be monitored by an eye care professional to prevent antibiotic resistance and potential side effects. If symptoms persist or worsen despite treatment, consult your doctor for alternative management strategies. Some studies suggest that erythromycin can be effective in treating blepharitis, especially when combined with lid hygiene and other treatments 3, 4, 5, 6. However, the most recent and highest quality study 2 supports the use of erythromycin eye ointment for 90 days as an initial treatment course. This study demonstrated a significant improvement in blepharitis-related signs and symptoms, with a very strong effect size. Therefore, based on the most recent evidence, a 90-day treatment course with erythromycin eye ointment is recommended for blepharitis associated with arthritis.
