Hydroxyurea Initiation and Management in Newly Diagnosed Children with Sickle Cell Disease
Offer hydroxyurea to every child with HbSS or Sβ⁰-thalassemia starting at 9 months of age, even without clinical symptoms, and initiate it immediately upon diagnosis if the child is already older than 9 months. 1
When to Start Hydroxyurea
- Standard initiation age: 9 months for HbSS or Sβ⁰-thalassemia 2
- Earlier initiation (6 months): Consider if there is evidence of severe hemolytic anemia, chronic organ damage, clinical complications, or based on parental preference 2
- If diagnosed after 9 months: Start immediately upon diagnosis
- For HbSC or Sβ⁺-thalassemia: Only initiate if severe disease manifestations (≥3 pain admissions per 2 years, acute chest syndrome, or avascular necrosis) 2
The evidence strongly supports early initiation. The BABY HUG trial demonstrated significant benefit when started presymptomatically in infants, with no unique adverse effects even when continued for more than a decade 1. Research shows that children starting hydroxyurea between 5-12 months of age had fewer hospitalizations, pain crises, and transfusions compared to those starting at older ages 3.
Pre-Treatment Evaluation
Obtain these baseline laboratory studies before starting hydroxyurea 2:
- Complete blood count with differential
- Absolute reticulocyte count
- Comprehensive metabolic panel with lactate dehydrogenase
- Percent fetal hemoglobin
Dosing Strategy
Initial Dosing
- Standard starting dose: 15-20 mg/kg/day orally once daily 2
- For HbSC or Sβ⁺-thalassemia with severe disease: Start at 15 mg/kg/day due to increased risk for cytopenia 2
- Alternative low-dose approach: 10-12 mg/kg/day can be effective and may be safer in populations with ethnic neutropenia 4
Dose Escalation
The goal is to achieve maximum tolerated dose through stepwise escalation, targeting modest but safe myelosuppression 5. Adjust doses periodically for weight gain.
Formulations Available
- Capsules (500 mg)
- Fast-dissolving tablets
- Compounded liquid 1
Monitoring Schedule
CBC and reticulocyte count monitoring 1:
- Every 1-3 months depending on:
- Duration of therapy
- Whether stable dose achieved
Fetal hemoglobin monitoring 2:
- At least once per year (or more frequently per clinician preference)
- Used to monitor medication effect and adherence
Expected Benefits
Hydroxyurea increases total and fetal hemoglobin and decreases 1:
- Vaso-occlusive pain episodes
- Acute chest syndrome
- Hospitalizations
- Need for transfusion
- Improves health-related quality of life
The mechanism works by elevating HbF, which delays sickle hemoglobin polymerization and reduces erythrocyte sickling 5. Additional benefits include lower neutrophil and platelet counts, reduced inflammation, and improved rheology.
Adverse Effects and Safety
Primary adverse effect: Myelosuppression (expected and actually therapeutic) 1
Common but mild toxicities 5:
- Modest cytopenia (predictable)
- Occasional gastrointestinal manifestations (well-tolerated)
- Cutaneous manifestations (well-tolerated)
Long-term risks: Mainly theoretical but require ongoing surveillance 5
Important safety considerations 6:
- Risk of cutaneous vasculitic toxicities
- Risk of secondary malignancies (leukemia, skin cancers)
- Teratogenic potential—females of reproductive potential need contraception
Handling Instructions for Caregivers
Caregivers should wear disposable gloves when handling capsules or bottles. Do not open capsules. If contact with crushed/opened capsules occurs on skin, wash immediately with soap and water. If eye contact occurs, flush for at least 15 minutes 6.
Family Education Timing
Begin discussions about hydroxyurea as early as the first visit (2 months of age) to discuss rationale, benefits, potential risks, and address concerns or barriers 2. This early education is critical for acceptance and adherence.
Common Pitfalls to Avoid
- Delaying initiation until symptoms appear: The evidence clearly shows presymptomatic initiation at 9 months prevents complications
- Underdosing: Aim for maximum tolerated dose through escalation, not just starting dose
- Inadequate monitoring: Regular CBC monitoring is essential to detect myelosuppression
- Not adjusting for weight gain: Periodic dose adjustments are necessary as children grow
- Failing to monitor adherence: Annual fetal hemoglobin levels help assess medication effect and adherence
Special Populations
Renal impairment: Reduce dose when creatinine clearance <60 mL/min or in ESRD following hemodialysis, as exposure increases by 64% 6
Stroke prevention context: Hydroxyurea can be used as an alternative to chronic transfusion therapy for primary stroke prevention, particularly when transfusions are not accessible or tolerable 7. The SPRING and SPHERE trials demonstrated effectiveness in reducing TCD velocities and stroke incidence comparable to historical controls.
