In a 79‑kg adult with type‑2 diabetes on metformin and ozempic who presents with right‑upper‑quadrant pain, yellow urine and stool, markedly elevated transaminases (AST ≈1400 U/L, ALT ≈2100 U/L) and cholestatic enzymes (ALP ≈700 U/L, GGT ≈900 U/L) with elevated bilirubin, and a positive hepatitis A RNA PCR, are these findings solely attributable to acute hepatitis A infection, or could autoimmune hepatitis also be present?

Waarom hepatitis A de volledige verklaring is

  • Hyperbilirubinemie (totaal bilirubine 43-64 µmol/L, direct 45-65 µmol/L)

Waarom AIH onwaarschijnlijk is

Autoantibody profiel spreekt tegen AIH

Alle klassieke AIH-geassocieerde autoantibodies zijn negatief 1, 2:

  • ANA: negatief
  • Anti-gladde spier (SMA): negatief
  • Anti-mitochondriaal (AMA): negatief
  • Anti-LKM: negatief
  • Anti-SLA/LP: negatief
  • Anti-LC-1: negatief

Metformine

  • Kan zeldzaam cholestasische, hepatocellulaire of gemengde leverinjury veroorzaken 4
  • Echter: patiënt gebruikt dit chronisch zonder eerdere leverproblemen
  • Temporele relatie ontbreekt (geen recente start/dosisverhoging)

Ozempic (semaglutide)

Differentiaaldiagnose overwegingen

Steatosis hepatis / MAFLD als "second hit"

  • Echo toont steatosis hepatis

Drug-induced liver injury (DILI)

  • Alle medicatie is gestopt zonder recidief

Praktisch beleid

Monitoring

  • Controle levertesten over 2 maanden zoals gepland is adequaat

Wanneer AIH alsnog overwegen?

Overweeg AIH alleen indien 1, 2:

Vaccinatie familie

  • Kinderen zijn via GGD gevaccineerd (goed)
  • Partner en huisgenoten: controleer HAV-immuniteit en vaccineer indien nodig
  • Patiënt komt uit Filipijnen waar HAV endemisch is

Conclusie diagnostiek

  • Negatieve autoantibodies
  • Spontane verbetering zonder immunosuppressie
