When to Order Soluble IL-2 Receptor (sCD25), Soluble IL-2, and CXCL19
Order soluble CD25 (sIL-2R) primarily when you suspect hemophagocytic lymphohistiocytosis (HLH), as it is a validated diagnostic criterion with levels ≥2400 U/mL being 93% sensitive and 100% specific in the pediatric dataset that established HLH-2004 criteria 1, 2.
Primary Clinical Indications for sCD25
Hemophagocytic Lymphohistiocytosis (HLH)
- sCD25 ≥2400 U/mL is one of the 8 diagnostic criteria for HLH 1
- Order when patient presents with:
- Fever + splenomegaly + cytopenias (≥2 lineages)
- Ferritin >10,000 mg/L (though ferritin >7,000-10,000 mg/L should trigger consideration)
- Hypertriglyceridemia and/or hypofibrinogenemia
- sCD25 outperforms ferritin diagnostically with area under curve of 0.90 vs 0.78 for ferritin 1
- Sensitivity is 89% for macrophage activation syndrome in juvenile idiopathic arthritis and 88% for secondary HLH in acute liver failure 2
Disease Activity Monitoring
Once HLH is diagnosed, sCD25 serves as a dynamic marker that falls with treatment response 2. Initial levels >10,000 U/mL indicate poor prognosis 2.
Sarcoidosis
sCD25 facilitates diagnosis and monitors disease activity in sarcoidosis, though specific cutoffs vary 3.
Rheumatoid Arthritis
- Order sCD25 when ESR and/or CRP are negative but clinical suspicion for active disease remains high 4
- In one study, 47.2% of RA patients with negative ESR/CRP had elevated sCD25, and 82.4% of these had DAS28 >3.2 4
- Specificity is 83% for RA diagnosis, though sensitivity is only 66.1% 4
- Correlates with ESR (r=0.321), CRP (r=0.446), and DAS28 score (r=0.324) 4
Monitoring IL-2 Receptor Antagonist Therapy
For patients receiving basiliximab or daclizumab (IL-2 receptor antagonists), monitor renal function, CBC counts, and infection 5, 6, 5, 6. This is a Grade 1C recommendation for monitoring infusion reactions and a Grade 1C recommendation for monitoring renal function, CBC, and infection 5.
Secondary Indications
Autoimmune Diseases
Consider ordering in:
- Systemic lupus erythematosus
- Adult-onset Still's disease
- ANCA-associated vasculitis
- IgG4-related disease
- Systemic sclerosis (reflects immune activation and treatment response) 7
Malignancies
sCD25 reflects tumor bulk and activity in:
- Hematological malignancies (leukemias, lymphomas)
- Some solid tumors (melanoma, renal, head/neck, esophageal, lung carcinomas) 8
Important Caveats
Do not order soluble IL-2 (the cytokine itself) or CXCL19 routinely in clinical practice - these are primarily research tools. The evidence provided focuses on sCD25 (soluble IL-2 receptor), which is the clinically validated test.
CXCL19 is a homeostatic chemokine involved in lymphocyte recruitment and lymphoid neogenesis 9, but it lacks established clinical utility as a diagnostic or monitoring biomarker in routine practice.
Practical Algorithm for Ordering sCD25
- Suspect HLH? (fever + splenomegaly + cytopenias + ferritin >3,000 mg/L) → Order sCD25
- Confirmed HLH? → Use sCD25 to monitor treatment response (should decrease)
- Active RA with negative inflammatory markers? → Order sCD25 to detect occult inflammation
- Patient on IL-2 receptor antagonist therapy? → Monitor per protocol (renal function, CBC, infection)
- Sarcoidosis or other autoimmune disease requiring activity assessment? → Consider sCD25 as adjunctive marker
The test is inexpensive and underutilized 2, making it particularly valuable when clinical suspicion is high but traditional inflammatory markers are equivocal or when monitoring specific immunosuppressive therapies.
