Response Assessment for SOX Regimen in Colorectal Cancer
Response assessment should be performed after every 2-3 cycles of the SOX regimen in metastatic colorectal cancer.
Timing of Response Assessment
While the provided evidence does not contain specific guidelines for the SOX (S-1 plus oxaliplatin) regimen itself, the standard practice for oxaliplatin-based combination chemotherapy in colorectal cancer provides clear guidance:
- Standard interval: Every 2-3 cycles 1, 2
- Imaging modality: Chest/abdomen/pelvis CT with contrast (or chest CT with abdominal/pelvic MRI with contrast) 1
- PET/CT should NOT be used for monitoring treatment response 1
Clinical Context
The SOX regimen follows the same assessment principles as other oxaliplatin-based doublets (FOLFOX, CAPEOX) used in colorectal cancer. Since SOX is administered every 3 weeks 3, 4, this translates to:
- First assessment: After 6-9 weeks (2-3 cycles)
- Subsequent assessments: Every 6-9 weeks until progression or unacceptable toxicity
Important Considerations
For adjuvant treatment (Stage III colon cancer):
- Response assessment occurs only after completion of therapy; do not repeat scans during treatment 2
- Standard duration is 8 cycles for SOX 4
For metastatic disease:
- Continue imaging every 2-3 cycles throughout treatment
- Treatment continues until disease progression or unacceptable toxicity 5
Oxaliplatin neurotoxicity management:
- Consider discontinuing oxaliplatin after 3-4 months (approximately 4-5 cycles of SOX) if significant neurotoxicity develops (≥ grade 2) 1, 2
- Other drugs (S-1 ± bevacizumab) can be maintained until progression
Common Pitfall
Do not wait longer than 3 cycles before the first assessment in metastatic disease, as this may delay recognition of progressive disease and unnecessarily expose patients to ineffective toxic therapy.
