What is Emtricitabine-Tenofovir (Truvada) Used For?
Emtricitabine-tenofovir (Truvada) is used for two distinct purposes: treating HIV-1 infection in combination with other antiretroviral medications, and preventing HIV-1 acquisition (pre-exposure prophylaxis or PrEP) in HIV-negative individuals at high risk of infection. 1
Treatment of HIV-1 Infection
When used to treat HIV-1, emtricitabine-tenofovir serves as the nucleoside/nucleotide reverse transcriptase inhibitor (NRTI) backbone of combination antiretroviral therapy (ART). It must be combined with additional antiretroviral drugs—it is not a complete HIV treatment regimen by itself. 2, 1
Key Treatment Applications:
- First-line therapy: Combined with integrase inhibitors (bictegravir, dolutegravir, elvitegravir) or other drug classes 2
- Adults and children: Approved for those 12 years and older weighing at least 77 pounds 1
- Goals of treatment: Reduces viral load (HIV-1 RNA), increases CD4+ T-cell counts, and improves immune function to reduce risk of opportunistic infections and death 1
The 2025 International Antiviral Society-USA guidelines confirm that tenofovir (either as TDF or TAF) plus emtricitabine or lamivudine remain the recommended NRTI backbone for initial ART regimens. 2
Pre-Exposure Prophylaxis (PrEP) for HIV Prevention
For HIV prevention, Truvada is used as PrEP in HIV-negative individuals at substantial risk of acquiring HIV-1 infection. This represents a fundamentally different use—preventing infection rather than treating it. 3, 1
Who Should Use PrEP:
- Men who have sex with men at high risk for HIV acquisition 3, 1
- Heterosexual serodiscordant couples (one partner HIV-positive, one HIV-negative) 3
- Anyone with ongoing substantial HIV exposure risk through sexual contact 1
Critical PrEP Requirements:
You must be confirmed HIV-negative before starting and while taking PrEP. 1 This is non-negotiable because:
- Taking Truvada alone when HIV-positive can lead to viral resistance 1
- HIV-positive individuals need a complete multi-drug regimen, not just two drugs 1
- Testing must occur at least every 3 months while on PrEP 3, 1
PrEP Effectiveness Depends on Adherence:
Recent pooled data from 6,296 cisgender women showed that adherence is the key determinant of PrEP effectiveness. Women with consistently daily adherence had zero HIV infections, while those with consistently high adherence (4-6 doses/week) had an incidence rate of only 0.13 per 100 person-years. In contrast, consistently low adherence (<2 doses/week) resulted in 1.27 infections per 100 person-years. 4 Missing doses substantially increases infection risk. 1
PrEP Must Be Part of Comprehensive Prevention:
Truvada alone does not guarantee HIV prevention. 1 It must be combined with:
- Consistent condom use
- Regular HIV and STI testing (at least every 3 months)
- Risk reduction counseling
- Treatment of other sexually transmitted infections 3
Additional Indication: Hepatitis B Co-infection
Emtricitabine-tenofovir also treats chronic hepatitis B virus (HBV) infection. 1, 5
Critical warning: If you have both HIV and HBV and stop taking Truvada, your hepatitis B can severely worsen. Close monitoring or alternative HBV treatment is essential if discontinuing. 1
Dosing
The standard dose is one tablet daily (emtricitabine 200 mg/tenofovir disoproxil fumarate 300 mg), taken orally with or without food. 3, 1 This applies to both HIV treatment and PrEP indications.
Important Safety Considerations
Contraindications for PrEP Use:
- Already HIV-positive (confirmed or unknown status) 1
- Creatinine clearance <60 mL/min 3
- Currently breastfeeding 3, 1
Monitoring Requirements:
- HIV testing before initiation and at least every 3 months 3, 1
- Renal function (creatinine clearance) at 3 months, then every 6 months 3
- Hepatitis B testing before starting 3
- STI screening every 6 months 3
- Pregnancy testing for women at each visit 3
Serious Risks:
- Lactic acidosis (rare but potentially fatal) 1
- Severe liver problems, especially in those with hepatitis B 1
- Kidney problems requiring monitoring 3, 1
- Bone density decreases 1
The safety profile is generally favorable—one analysis concluded that Truvada for PrEP compares favorably to aspirin in terms of user safety. 6
