Ceftriaxone (Rocephin) IM Dosing for 20.64 kg Child
For a child weighing 20.64 kg, administer ceftriaxone 1,032 mg (approximately 1 gram) intramuscularly as a single daily dose for most serious infections, or 1,548 mg (approximately 1.5 grams) divided into two doses for meningitis.
Standard Dosing by Indication
For Skin/Soft Tissue Infections
- Dose: 50-75 mg/kg once daily 1
- For this 20.64 kg child: 1,032-1,548 mg once daily
- Maximum daily dose: 2 grams
- Practical administration: Give 1 gram IM once daily
For Acute Otitis Media
- Single dose: 50 mg/kg (not to exceed 1 gram) 1
- For this child: 1,032 mg as a single IM injection
- This is a one-time treatment, highly effective per clinical trials 2
For Serious Infections (Non-Meningitis)
- Dose: 50-75 mg/kg/day divided every 12 hours 1
- For this child: 516-774 mg every 12 hours OR 1,032-1,548 mg once daily
- Maximum: 2 grams daily
- Practical administration: 500-750 mg every 12 hours OR 1 gram once daily
For Meningitis
- Initial dose: 100 mg/kg (not to exceed 4 grams) 1
- For this child: 2,064 mg initially (give 2 grams)
- Maintenance: 100 mg/kg/day divided every 12 hours or once daily 1
- For this child: 1,032 mg every 12 hours OR 2,064 mg once daily
- Duration: 7-14 days
- Recent evidence favors once-daily dosing for better CSF penetration 3
Reconstitution for IM Administration
To prepare 1 gram dose:
- Add 3.6 mL diluent to 1 gram vial
- Final concentration: 250 mg/mL
- Draw up 4 mL for 1,000 mg dose 1
Alternative concentration:
- Add 2.1 mL diluent to 1 gram vial
- Final concentration: 350 mg/mL
- Draw up 2.9 mL for 1,000 mg dose 1
Critical Administration Points
- Inject deep into large muscle mass (gluteus or lateral thigh) 1
- Aspirate before injection to avoid intravascular administration 1
- IM ceftriaxone is highly effective even in serious infections when IV access is difficult 4, 5, 6
- Clinical studies demonstrate 94-97% cure rates with IM administration 5, 6
Key Clinical Considerations
Once-daily dosing is preferred for most indications because:
- Achieves adequate tissue penetration 7, 8, 7
- Better CSF penetration for meningitis 3
- Improved compliance
- Allows outpatient therapy 5, 6
Twice-daily dosing should be considered for:
- Confirmed meningitis (though once-daily is now supported by recent data) 3
- Severe pneumonia requiring mechanical ventilation 9
- Infections with organisms having MIC >2 mg/L 10
