OPD Mein Bukhar Ka Approach Aur Treatment
OPD mein bukhar wale mareez ko dekhte waqt sabse pehle temperature ko oral ya rectal thermometer se accurately measure karein (axillary ya tympanic membrane thermometer kam reliable hain), aur phir systematic clinical evaluation karke infection ki source dhoondhein - respiratory, urinary, abdominal ya skin/soft tissue. 1
Temperature Measurement Ka Sahi Tareeqa
- Oral ya rectal digital thermometer use karein - yeh sabse zyada accurate hain
- Axillary, tympanic membrane, temporal artery thermometers kam reliable hain aur avoid karein 1
- Fever ki definition: Single temperature >38.3°C (101°F) 1
- Elderly patients mein >37.8°C oral ya baseline se 1.1°C increase bhi significant hai 2
Systematic Clinical Evaluation - Kya Dekhna Hai
History Mein Zaroor Poochein:
- Bukhar ki duration aur pattern - acute vs chronic
- Associated symptoms:
- Respiratory: cough, sore throat, shortness of breath, chest pain
- Urinary: dysuria, frequency, urgency, flank pain
- GI: diarrhea, abdominal pain, vomiting
- Neurological: headache, altered mental status, neck stiffness
- Recent travel history - especially tropical areas (malaria, typhoid, dengue ke liye) 3
- Recent surgery - especially thoracic, abdominal, pelvic 1
- Immunocompromised status - chemotherapy, steroids, HIV
- Drug history - antibiotics (C. difficile risk), drug fever possibility 4
Physical Examination - Systematic Approach:
Vital signs check karein pehle:
- Temperature, heart rate, blood pressure, respiratory rate
- Tachycardia aur hypotension sepsis indicate kar sakte hain
Phir systematically examine karein:
- Mental status - confusion ya altered sensorium serious infection suggest karta hai
- Oropharynx - pharyngitis, tonsillitis, dental abscess
- Conjunctiva - conjunctivitis
- Chest examination - pneumonia ke signs (crackles, bronchial breathing, dullness)
- Cardiovascular - new murmur (endocarditis)
- Abdomen - tenderness, organomegaly, peritoneal signs
- Skin examination - rashes, cellulitis, abscess, pressure ulcers
- Perineum aur perirectal area - abscess, infection 4, 5
- Indwelling devices - IV lines, catheters (infection source)
Initial Investigations - OPD Mein Kya Order Karein
Basic Tests (Sabhi Febrile Patients Ke Liye):
- Complete blood count - leukocytosis/leukopenia check karein
- Urinalysis - UTI rule out karne ke liye (common cause)
- Blood cultures - agar serious infection suspect ho
- Chest X-ray - respiratory symptoms ya signs ho toh 1, 4
Additional Tests (Clinical Suspicion Ke Basis Par):
Agar respiratory symptoms hain:
- Sputum culture aur Gram stain
- COVID-19 PCR (community transmission ke basis par) 1
Agar diarrhea hai:
- Stool examination
- C. difficile toxin assay - agar recent antibiotics liye hain (60 days ke andar) 4
Agar UTI suspect hai:
- Urine culture aur sensitivity
Agar enteric fever (typhoid) suspect hai:
- Blood culture (pehle week mein highest yield)
- Widal test unreliable hai, avoid karein 3
Agar bacterial infection ka probability low-to-intermediate hai:
- Procalcitonin (PCT) ya C-reactive protein (CRP) measure kar sakte hain clinical evaluation ke saath 1
- Caveat: Agar bacterial infection ka probability high hai, toh PCT/CRP se bacterial infection rule out mat karein 1
Treatment Approach - Kab Kya Karein
Antipyretics Ka Use:
Important principle: Sirf temperature kam karne ke liye routinely antipyretics mat dein 1
Antipyretics tab dein jab:
- Patient ko discomfort ho
- Patient comfort chahta ho
- Pain relief ke liye 1
Preferred agents:
Non-pharmacological methods se zyada pharmacological antipyretics prefer karein comfort ke liye 1
Antibiotics Ka Decision - Sabse Critical Point:
Jab clinical evaluation suggest kare ki infection hai aur patient seriously ill hai ya deteriorating hai, toh cultures lene ke TURANT BAAD empirical antibiotics start karein 4
Antibiotic selection depend karta hai:
- Suspected source of infection par
- Community vs hospital-acquired infection
- Local antibiotic resistance patterns par 4
Specific Scenarios:
Community-acquired pneumonia suspect hai:
- Amoxicillin-clavulanate ya macrolide (azithromycin)
UTI suspect hai:
- Fluoroquinolone ya cephalosporin (culture sensitivity ke basis par adjust karein)
Enteric fever (typhoid) suspect hai:
- First-line: IV Ceftriaxone (fluoroquinolone resistance common hai Asia se) 3
- Oral alternative: Azithromycin (uncomplicated cases mein)
- Treatment duration: 14 days (relapse kam karne ke liye) 3
Skin/soft tissue infection:
- Flucloxacillin ya amoxicillin-clavulanate
- Agar MRSA risk hai: Add doxycycline ya clindamycin
Common Pitfalls - In Se Bachein:
Fever phobia: Har bukhar ko treat karna zaruri nahi - comfort-based approach use karein 7
Unnecessary antibiotics: Viral infections mein antibiotics mat dein - clinical judgment use karein
Inadequate source identification: Systematic examination kiye bina antibiotics start mat karein
Delayed treatment in sepsis: Agar sepsis suspect hai, 1 hour ke andar antibiotics start karein 4
Ignoring drug fever: Agar koi obvious source nahi mil raha aur patient recently antibiotics ya drugs le raha hai, drug fever consider karein 4
Follow-up Aur Monitoring:
Outpatient management ke liye criteria:
- Hemodynamically stable ho
- Oral intake adequate ho
- Reliable follow-up possible ho
- 24 hours mein reassessment plan ho 8, 9
Red flags - Turant hospital refer karein:
- Altered mental status
- Hypotension (systolic BP <90 mmHg)
- Respiratory distress
- Capillary refill time >3 seconds 6
- Dehydration signs
- Immunocompromised patients with high fever
Follow-up plan:
- 24-48 hours mein reassess karein
- Culture results check karein aur antibiotics adjust karein
- Agar 48-72 hours mein improvement nahi hai, re-evaluate karein aur imaging consider karein 8
