Visual Side Effects of Tadalafil
Tadalafil can cause visual disturbances including color vision changes (particularly blue/green discrimination difficulties), blurred vision, and rarely, serious vision-threatening conditions such as non-arteritic anterior ischemic optic neuropathy (NAION) and central serous chorioretinopathy (CSCR).
Common Visual Side Effects
The most frequently reported visual side effects are generally mild and transient:
- Color vision changes occur in less than 0.1% of patients across all clinical studies 1. These typically manifest as a blue tinge to objects or difficulty distinguishing between blue and green colors 2
- Blurred vision is reported as an uncommon adverse event 2
- Conjunctival hyperemia (eye redness), eye pain, and increased tearing may occur 2
These common effects are related to tadalafil's inhibition of PDE6 in the retina, which is involved in phototransduction. Single oral doses have demonstrated transient dose-related impairment of color discrimination, with peak effects occurring near the time of peak plasma levels 2.
Serious Vision-Threatening Complications
Non-Arteritic Anterior Ischemic Optic Neuropathy (NAION)
NAION represents the most serious visual complication and can result in permanent vision loss 2. This condition has been reported rarely in postmarketing surveillance in temporal association with PDE5 inhibitor use, including tadalafil.
Risk factors for NAION include:
- Low cup-to-disc ratio ("crowded disc")
- Age over 50 years
- Diabetes
- Hypertension
- Coronary artery disease
- Hyperlipidemia
- Smoking 2
A case report documented NAION in a 42-year-old healthy male taking tadalafil, resulting in inferior altitudinal visual field loss that did not improve after discontinuation 3. This underscores that NAION can occur even in younger patients without traditional risk factors.
Central Serous Chorioretinopathy (CSCR)
CSCR should be considered a class-specific side effect of PDE5 inhibitors, including tadalafil 4. Multiple case reports demonstrate:
- Serous macular detachments and pigment epithelial detachments 5
- Increased choroidal thickness 5
- Resolution of symptoms and subretinal fluid within 3 months of discontinuing tadalafil 5
- Positive rechallenge reactions (symptoms return when medication is restarted) 4
Retinal Photoreceptor Damage
One case report documented reversible damage to parafoveal photoreceptors using spectral domain optical coherence tomography, showing hyperreflective changes in the photoreceptor internal segment-outer segment interface that mimicked serous retinal detachment. These changes resolved 2 months after discontinuing tadalafil 6.
Clinical Implications and Monitoring
Patients should be instructed to immediately discontinue tadalafil and seek urgent medical attention if they experience:
- Sudden decrease or loss of vision in one or both eyes
- Visual field defects
- Persistent blurred vision or color vision changes
Important caveats:
- It is uncertain whether these vision-threatening events are directly caused by PDE5 inhibitors, the underlying vascular risk factors, sexual activity itself, or a combination of factors 2
- Most patients who developed NAION had underlying anatomic or vascular risk factors, but cases have occurred in younger, healthier individuals 3
- The temporal association between tadalafil use and serious visual events warrants caution, even though causality cannot be definitively established
Long-Term Safety Data
A 6-month study of daily tadalafil (5 mg) or sildenafil (50 mg) versus placebo found no significant differences in electroretinography parameters, visual acuity, color discrimination, visual field testing, or intraocular pressure 7. This suggests that daily use at therapeutic doses does not cause cumulative retinal toxicity in most patients. However, this does not exclude the risk of rare idiosyncratic reactions like NAION or CSCR.
Prescribing physicians should counsel patients about these visual risks before initiating tadalafil, particularly in those with pre-existing vascular risk factors 1, 5.
