Rizatriptan Second Dose Timing
A second dose of rizatriptan may be taken 2 hours after the first dose if the headache returns or persists 1.
Dosing Algorithm for Adults
Initial Dose Response Assessment
- Evaluate headache response at 2 hours after the first dose
- Response is defined as reduction from moderate/severe pain to mild/no pain
Second Dose Criteria
You may take a second dose if:
- Your headache comes back after initial improvement (recurrence)
- Your headache persists with moderate or severe pain at 2 hours
Timing requirements:
Maximum Daily Limits
- Do not exceed 30 mg total in 24 hours 1
- Example: No more than three 10 mg tablets per day
- Example: No more than six 5 mg tablets per day
Important Caveats
Evidence on Second Dose Efficacy
The FDA label explicitly states that second doses were allowed "2 to 24 hours after dosing for treatment of recurrent headache" in clinical trials 1. However, a second dose for partial or no response at 2 hours has been shown in randomized controlled trials to be ineffective 2. The second dose is primarily effective for true headache recurrence (when pain returns after initial relief), not for inadequate initial response.
Pediatric Patients (Ages 6-17)
It is not known if taking more than 1 dose of rizatriptan in 24 hours is safe and effective in children 1. If your child's headache does not improve or returns, contact their doctor rather than administering a second dose.
Special Consideration with Propranolol
If you take propranolol, your dosing may need adjustment—consult your prescribing physician as drug interactions can affect rizatriptan levels 1.
Clinical Context from Guidelines
The 2025 American College of Physicians guideline recommends combination therapy of rizatriptan with either an NSAID or acetaminophen for better outcomes, particularly in patients with inadequate response to monotherapy 3. This combination approach may reduce the need for second doses by improving initial response rates.
