Desloratadine Dosing Per Kilogram Body Weight
Desloratadine is not dosed per kilogram body weight in clinical practice; it is dosed by age-based fixed dosing regimens. The FDA-approved dosing for desloratadine uses age-stratified fixed doses rather than weight-based calculations 1.
FDA-Approved Dosing by Age
The standard dosing regimen for desloratadine is:
- Adults and children ≥12 years: 5 mg once daily
- Children 6-11 years: 2.5 mg once daily
- Children 2-5 years: 1.25 mg once daily
- Children 6 months to <2 years: 1 mg once daily (for 6-11 months) or 1.25 mg once daily (for 1-2 years)
These doses were established through pharmacokinetic studies demonstrating that fixed age-based dosing achieves similar systemic exposure (AUC) across age groups compared to the adult 5 mg dose 2, 3.
Why Not Weight-Based Dosing?
Unlike many pediatric medications, desloratadine dosing was specifically designed around age rather than weight because:
- Pharmacokinetic modeling showed that age-stratified fixed doses produce comparable drug exposure to adults receiving 5 mg, without requiring weight-based calculations 2, 3
- The apparent clearance (CL/F) varies by age group but the fixed doses compensate appropriately: children 6 months-1 year have CL/F of 27.8 L/h, children 1-2 years have 35.5 L/h, compared to adults at 137 L/h 2
- Clinical trials validated efficacy and safety using these age-based fixed doses rather than mg/kg dosing 3, 4
Special Circumstances
For chronic spontaneous urticaria not responding to standard doses: International guidelines (EAACI/GA²LEN/EDF/APAAACI) recommend up-dosing to 2-4 times the standard dose 5, 6. This means:
- Adults may receive 10-20 mg daily (still not calculated per kg)
- Studies confirm 10 mg daily is safe and effective for refractory urticaria 5
Hepatic impairment: Reduce to 5 mg every other day in adults 1
No renal dose adjustment is required based on FDA labeling 1.
Common Pitfall to Avoid
Do not attempt to calculate desloratadine doses using mg/kg formulas from other antihistamines. The drug was specifically studied and approved using fixed age-based dosing, and this approach has been validated to achieve appropriate therapeutic exposure across pediatric and adult populations 2, 3, 4.
