No Injectable Formulation of Vraylar Exists
Vraylar (cariprazine) is only available as oral capsules in 1.5 mg, 3 mg, 4.5 mg, and 6 mg strengths—there is no FDA-approved injectable formulation of this medication. 1
Available Formulation
The FDA-approved drug label explicitly states that Vraylar comes exclusively as hard gelatin capsules for oral administration. The capsules contain cariprazine hydrochloride powder equivalent to 1.5,3,4.5, or 6 mg of cariprazine base 1.
Clinical Implications
- No intramuscular (IM) option exists for acute agitation or patients unable to take oral medications
- No long-acting injectable (LAI) formulation is available, despite LAI antipsychotics being recommended for patients with poor adherence 2
- If injectable antipsychotic therapy is needed, you must switch to a different agent that offers IM or LAI formulations (such as risperidone, paliperidone, olanzapine, or aripiprazole)
Important Pharmacokinetic Consideration
Even if an injectable form existed, cariprazine's unique pharmacology would complicate its use: the drug has an extremely long half-life due to active metabolites (didesmethylcariprazine), with plasma concentrations declining by only 50% over approximately one week after discontinuation 1. This extended duration makes oral dosing practical but would create challenges for injectable formulation development.
Research Context
While one research study explored intranasal lipid-based nanocarriers for cariprazine delivery to the brain 3, this remains purely investigational and has no clinical availability. This was a formulation development study, not an approved product.
