Can Laser Eye Surgery Cause Migraine Headaches?
Migraine headaches are not listed as a recognized complication of laser eye surgery (LASIK, PRK, or other refractive procedures) in current evidence-based guidelines. The most comprehensive and recent refractive surgery guidelines do not identify migraine as a side effect or complication of these procedures 1.
What the Evidence Shows
The 2023 American Academy of Ophthalmology Refractive Surgery Preferred Practice Pattern® provides an exhaustive list of known complications following LASIK and other laser refractive procedures. This list includes 1:
- Visual complications: glare, halos, decreased contrast sensitivity, irregular astigmatism
- Ocular surface issues: dry eye symptoms, decreased corneal sensitivity, corneal neuralgia
- Structural complications: flap striae, epithelial ingrowth, diffuse lamellar keratitis, corneal ectasia
- Other complications: ptosis, corticosteroid-induced complications, recurrent corneal erosion
Notably absent from this comprehensive list is migraine or headache of any type.
The 2025 guidelines for keratorefractive lenticule extraction surgery similarly catalog complications extensively but make no mention of migraine headaches as a consequence 2.
Important Clinical Distinction
While one older case report from 1996 describes two patients with pre-existing migraine history who experienced migraine attacks after ocular surgery 3, this represents:
- Patients with known migraine susceptibility
- Migraine triggered by the stress/trauma of surgery (not caused by the procedure itself)
- A phenomenon that could occur after any ocular procedure, not specific to laser refractive surgery
This is fundamentally different from laser surgery causing migraines in patients without prior history.
Clinical Implications
If a patient develops headaches after laser eye surgery, consider alternative explanations:
- Refractive error: Residual undercorrection or overcorrection causing eye strain
- Dry eye disease: A common post-LASIK complication that can cause discomfort and headache-like symptoms 1
- Corneal neuralgia: Chronic neuropathic pain that may be mischaracterized as headache 1
- Coincidental migraine: Unrelated to the procedure
- Pre-existing migraine: Triggered by surgical stress in susceptible individuals 3
Bottom Line
Based on the highest quality guideline evidence from 2023-2025, there is no established causal relationship between laser eye surgery and migraine headaches. The extensive complication profiles documented in authoritative guidelines do not include migraine as a recognized adverse outcome 1.
