Diagnostic Workup for Eosinophilia with Thrombocytosis
The combination of eosinophilia and elevated platelets requires immediate evaluation for myeloid/lymphoid neoplasms with tyrosine kinase (TK) fusion genes, particularly FIP1L1-PDGFRA, as this represents a potentially life-threatening but treatable hematologic malignancy. 1
Initial Clinical Assessment
Focus your history on:
- Travel exposure (parasitic infections, especially Strongyloides)
- New medications (drug-induced eosinophilia)
- Recurrent infections (immunodeficiency syndromes)
- Symptoms of organ involvement: weakness, fatigue, cough, dyspnea, myalgias, angioedema, rash, fever, rhinitis 1
Physical examination must assess:
- Skin lesions (eczema, urticaria, rash)
- Hepatosplenomegaly (suggests myeloproliferative disease)
- Cardiac findings (endomyocardial involvement is common with FIP1L1-PDGFRA) 1
Essential Laboratory Workup
First-tier testing 1:
- CBC with differential and peripheral blood smear review for dysplasia, monocytosis, circulating blasts
- Comprehensive metabolic panel with uric acid, LDH, liver function tests
- Serum tryptase (elevated in myeloproliferative variants, especially PDGFRA-associated disease)
- Vitamin B12 (elevated in myeloproliferative variants)
- Serum IgE levels
Second-tier testing based on clinical context 1:
- Strongyloides serology and stool ova/parasites
- ANCA and ANA (for eosinophilic granulomatosis with polyangiitis)
- Quantitative immunoglobulins
- Aspergillus IgE (if pulmonary symptoms)
Critical Diagnostic Studies
Bone marrow evaluation is mandatory to exclude primary (clonal) eosinophilia 1:
- Aspirate and biopsy with IHC for CD117, CD25, tryptase
- Reticulin/collagen stains for fibrosis
- Conventional cytogenetics
- FISH and/or nested RT-PCR for TK fusion gene rearrangements (FIP1L1-PDGFRA, PDGFRB, FGFR1, JAK2)
- Flow cytometry if lymphocytic variant suspected
The combination of thrombocytosis with eosinophilia is particularly concerning for myeloproliferative neoplasms, as these blood count abnormalities (neutrophilia, basophilia, thrombocytosis, monocytosis) commonly occur together in TK fusion gene-associated diseases 1.
Organ Damage Assessment
Since endomyocardial thrombosis and fibrosis are frequently documented in primary eosinophilic disorders, particularly with FIP1L1-PDGFRA 1:
- Cardiac imaging (echocardiography, cardiac MRI if indicated)
- Pulmonary function tests if respiratory symptoms
- Imaging studies for other organ involvement based on symptoms
Diagnostic Algorithm
Rule out secondary causes first 1, 2:
- Allergic disorders (most common, ~80% of cases)
- Parasitic infections (especially Strongyloides)
- Drug reactions
- Solid tumors or lymphomas (Hodgkin lymphoma, T-cell/B-cell lymphomas)
If secondary causes excluded, proceed to bone marrow evaluation for:
- Myeloid/lymphoid neoplasms with TK fusion genes
- Chronic eosinophilic leukemia, NOS
- Systemic mastocytosis (elevated tryptase is a key clue)
Consider NGS myeloid mutation panels if no TK fusion genes detected but clonality suspected 1
Idiopathic HES is a diagnosis of exclusion only after ruling out all primary and secondary causes 1
Critical Pitfalls
- Do not delay bone marrow evaluation if tryptase or B12 are elevated, as this strongly suggests myeloproliferative disease 1
- Thrombocytosis combined with eosinophilia increases suspicion for clonal disease and should not be dismissed as reactive 1, 3
- Cardiac evaluation is non-negotiable given the risk of life-threatening endomyocardial fibrosis in FIP1L1-PDGFRA-positive disease 1
- Recent evidence suggests basophilia and eosinophilia together in myeloproliferative neoplasms may indicate worse prognosis and disease transformation risk 3
Management Considerations
Treatment depends entirely on the underlying cause. If FIP1L1-PDGFRA is identified, imatinib is highly effective and potentially curative 1. For secondary causes, treat the underlying condition. Do not empirically treat with corticosteroids before completing the diagnostic workup, as this can mask clonal disease and delay appropriate targeted therapy.
