Dapagliflozin in CKD: Dosing and Clinical Implementation
For adults with CKD, initiate dapagliflozin 10 mg once daily if eGFR ≥25 mL/min/1.73 m² and continue even if eGFR subsequently falls below 25 mL/min/1.73 m² 1.
Initiation Criteria
Strong indications (1A recommendation):
- eGFR ≥20 mL/min/1.73 m² with urine ACR ≥200 mg/g (≥20 mg/mmol) 2
- Heart failure, regardless of albuminuria level 2
- Type 2 diabetes with eGFR ≥20 mL/min/1.73 m² 2
Weaker indication (2B recommendation):
- eGFR 20-45 mL/min/1.73 m² with urine ACR <200 mg/g 2
The 2024 KDIGO and BMJ guidelines provide risk-stratified recommendations, with stronger evidence supporting use in patients at very high risk of CKD progression (high certainty evidence for 48 fewer deaths per 1000 and 58 fewer kidney failure events per 1000) 3.
Dosing Algorithm
For CKD (non-glycemic indications):
- Standard dose: 10 mg once daily 1
- Initiate if eGFR ≥25 mL/min/1.73 m² 1
- Do NOT initiate if eGFR <25 mL/min/1.73 m² 1
- Continue 10 mg daily if eGFR falls below 25 mL/min/1.73 m² after initiation 1
For glycemic control in type 2 diabetes:
- Start 5 mg once daily, may increase to 10 mg for additional glycemic control 1
- Not recommended for glycemic control if eGFR <45 mL/min/1.73 m² (ineffective for glucose lowering) 1
Pre-Initiation Assessment
Before starting dapagliflozin, you must:
- Assess renal function (eGFR and urine albumin-to-creatinine ratio) 1
- Evaluate volume status - correct volume depletion before initiating 1
Monitoring Parameters
The reversible eGFR decline upon initiation is NOT an indication to discontinue 2. This is a critical pitfall to avoid - the initial eGFR dip (typically 2-4 mL/min/1.73 m²) represents hemodynamic changes and does not require dose adjustment or discontinuation 4.
Standard CKD monitoring frequency does not need to change with SGLT2i initiation 2. Continue routine monitoring as clinically indicated based on CKD stage.
Contraindications and Temporary Holds
Absolute contraindication:
- History of serious hypersensitivity reaction to dapagliflozin (anaphylaxis, angioedema) 1
Temporary withholding (≥3 days if possible):
- Major surgery or procedures with prolonged fasting 1
- Critical medical illness where ketosis risk is elevated 2
- Resume when clinically stable and oral intake resumed 1
Not recommended for CKD treatment:
- Polycystic kidney disease 1
- Patients requiring or with recent immunosuppressive therapy for kidney disease 1
Clinical Outcomes Evidence
The DAPA-CKD trial demonstrated a 39% reduction in the composite of ≥50% eGFR decline, ESKD, or renal/cardiovascular death (HR 0.61,95% CI 0.51-0.72, NNT=19) 5. Benefits were consistent regardless of diabetes status, across all age groups including octogenarians, and across the frailty spectrum 6, 7.
In stage 4 CKD specifically (eGFR <30 mL/min/1.73 m²), dapagliflozin showed a 27% reduction in the primary endpoint with similar safety profile 4. The eGFR slope benefit was 1.23 mL/min/1.73 m²/year slower decline compared to placebo 4.
Common Pitfalls to Avoid
- Don't stop for initial eGFR dip - this hemodynamic effect is expected and beneficial long-term 2
- Don't withhold in elderly patients - benefits extend to patients ≥80 years without increased adverse events 7
- Don't confuse glycemic and CKD dosing thresholds - eGFR ≥25 for CKD indication vs ≥45 for glycemic control 1
- Don't discontinue when eGFR falls below 25 - continue if already established on therapy 1
