Weight Gain Profile of Intuniv, Trileptal, and Latuda

Latuda (lurasidone) is weight-neutral and does not cause clinically significant weight gain, while Trileptal (oxcarbazepine) does cause significant weight gain similar to valproate, and Intuniv (guanfacine) has minimal to no association with weight gain.

Latuda (Lurasidone) - Weight Neutral

Lurasidone is among the most weight-neutral antipsychotics available 1. The 2017 Gastroenterology guideline explicitly states that "lurasidone and ziprasidone appear to be the most weight-neutral in the class" of antipsychotics 1.

Clinical Trial Data:

  • Adults with schizophrenia: Mean weight change of only +0.43 kg versus -0.02 kg for placebo 2
  • Only 4.8% of patients experienced ≥7% weight increase (compared to 3.3% on placebo) 2
  • Adolescents with schizophrenia: Mean weight change +0.5 kg versus +0.2 kg placebo 2
  • Bipolar depression (adults): Mean weight change +0.29 kg versus -0.04 kg placebo 2
  • Pediatric bipolar depression (10-17 years): Mean weight change +0.7 kg versus +0.5 kg placebo 2

Long-term Data:

Real-world analysis showed lurasidone was associated with weight loss of -0.77 kg at 1 year 3. Patients switching from higher-risk antipsychotics (olanzapine, quetiapine, risperidone, clozapine) lost an average of -1.68 kg 3. A 12-month pooled analysis showed mean weight change of only -0.4 kg with lurasidone, compared to +2.6 kg with risperidone and +1.2 kg with quetiapine 4.

Trileptal (Oxcarbazepine) - Causes Weight Gain

Oxcarbazepine causes significant weight gain comparable to valproate, though it is often overlooked in clinical practice. While the 2017 guideline states that "carbamazepine is generally included" in the category of antiepileptics associated with weight gain 1, oxcarbazepine (its structural derivative) has similar effects.

Clinical Evidence:

A prospective pediatric study demonstrated that oxcarbazepine monotherapy resulted in significant weight gain over 8 months 5:

  • Body weight, weight SDS, BMI, and BMI SDS all increased significantly
  • 15.4% of children became overweight or obese during treatment
  • The weight gain was similar to that observed with valproate (21.2% became overweight/obese) 5

This is clinically important because oxcarbazepine is often chosen as an alternative to carbamazepine with the assumption of better tolerability, but the weight gain profile is problematic.

Intuniv (Guanfacine Extended-Release) - Minimal Weight Impact

Guanfacine is not associated with clinically significant weight gain. The 2019 AAP ADHD guideline notes that adverse effects of extended-release guanfacine include somnolence, dry mouth, dizziness, irritability, headache, bradycardia, hypotension, and abdominal pain—but weight gain is not listed 6.

The 2022 pharmacotherapy review confirms that guanfacine's adverse effects are primarily cardiovascular (hypotension/bradycardia) and sedation-related, without mention of weight gain as a concern 7.

Important Caveat:

While guanfacine itself doesn't cause weight gain, its sedative effects could theoretically reduce activity levels. However, this is not supported by clinical trial data showing weight gain as a significant adverse effect.

Clinical Implications

When weight is a concern:

  • Choose Latuda among antipsychotics—it has the best weight profile and may even promote weight loss when switching from other agents 1, 3
  • Avoid Trileptal if weight gain is problematic; consider alternatives like lamotrigine, levetiracetam, or phenytoin which are weight-neutral 1
  • Intuniv is safe from a weight perspective and can be used without concern for metabolic effects

Monitor patients on Trileptal for weight gain with the same vigilance as those on valproate, particularly in children where the impact on growth and development is more pronounced 5. Consider counseling on healthy lifestyle adaptations at treatment initiation.
