Differential Diagnosis for 39 yo Female with Dry Non-Productive Cough and Chills

Single Most Likely Diagnosis

  • Viral Upper Respiratory Tract Infection: Given the symptoms of dry non-productive cough and chills, this is the most likely diagnosis. The recent history of chills 2 days ago also supports a viral etiology, which is common in the community and often self-limiting.

Other Likely Diagnoses

  • Postoperative Pneumonia: Considering the patient had a complicated cervical laminectomy in April 2025, there's a risk of developing pneumonia, especially if the patient had a prolonged hospital stay or required mechanical ventilation. The blood transfusion could also be a factor if the patient developed transfusion-related acute lung injury (TRALI), although this would typically present sooner.
  • Asthma or Reactive Airway Disease: The patient's history of PCOS might not directly relate to respiratory issues, but the presence of hypertension (HTN) could indicate a predisposition to other chronic conditions. An asthma exacerbation or reactive airway disease could present with a dry cough, especially if triggered by recent viral infections or environmental factors.
  • Acute Bronchitis: This condition often follows an upper respiratory infection and can present with a dry, non-productive cough. It's more likely in smokers or those with exposure to irritants, but can also occur in non-smokers, especially after a viral infection.

Do Not Miss Diagnoses

  • Pulmonary Embolism (PE): Given the patient's recent surgery and blood transfusion, there's an increased risk of deep vein thrombosis (DVT) and subsequently PE. Although the presentation is typically with sudden onset dyspnea, chest pain, or cough, a high index of suspicion is required in postoperative patients.
  • Tuberculosis (TB): Although less likely, TB can present with a chronic cough and systemic symptoms like chills. The patient's recent hospitalization and potential exposure to TB in the healthcare setting, along with the immunosuppressive effects of blood transfusion, make this a diagnosis not to miss.
  • Pneumonia due to Atypical Organisms: Mycoplasma, Legionella, or Chlamydophila pneumonia can present with similar symptoms and might not be as responsive to typical antibiotic treatments. These organisms can cause severe disease, especially in patients with underlying conditions.

Rare Diagnoses

  • Sarcoidosis: This autoimmune condition can present with respiratory symptoms, including a dry cough, but is less common and typically associated with other systemic symptoms or findings on imaging studies.
  • Lymphangitic Carcinomatosis: In the context of a recent surgery and potential for metastatic disease (though not directly mentioned), this rare condition involves the spread of cancer to the lymphatic vessels of the lung, presenting with respiratory symptoms. However, this would be extremely rare without a known primary malignancy.
