How Trelegy Works
Trelegy is a single-inhaler triple therapy that combines three distinct mechanisms of action: fluticasone furoate (an inhaled corticosteroid) reduces airway inflammation, umeclidinium (a long-acting muscarinic antagonist) blocks acetylcholine receptors to relax airway smooth muscle, and vilanterol (a long-acting beta2-agonist) stimulates beta2-adrenergic receptors to further relax airway smooth muscle 1.
Three-Component Mechanism
Fluticasone Furoate (Inhaled Corticosteroid)
- Anti-inflammatory action: Reduces chronic airway inflammation characteristic of COPD
- Works by suppressing inflammatory cells and mediators in the airways
- Particularly effective when combined with bronchodilators for preventing exacerbations 2
Umeclidinium (Long-Acting Muscarinic Antagonist - LAMA)
- Anticholinergic bronchodilation: Blocks muscarinic receptors (M3 subtype) in airway smooth muscle
- Prevents acetylcholine-mediated bronchoconstriction
- Provides sustained bronchodilation for 24 hours with once-daily dosing 3
Vilanterol (Long-Acting Beta2-Agonist - LABA)
- Beta-adrenergic bronchodilation: Stimulates beta2-adrenergic receptors on airway smooth muscle
- Activates adenylyl cyclase, increasing cyclic AMP, which leads to smooth muscle relaxation
- Provides complementary bronchodilation to the LAMA component 3
Synergistic Triple Action
The combination provides superior efficacy compared to individual components or dual therapy:
- Dual bronchodilation (LAMA + LABA): The two bronchodilators work through different receptor pathways, providing more complete airway smooth muscle relaxation than either alone 3
- Anti-inflammatory protection (ICS): The corticosteroid component reduces the underlying inflammatory process and exacerbation risk, particularly in patients with frequent exacerbations 2
- Complementary mechanisms: Bronchodilation improves airflow immediately while anti-inflammatory effects reduce long-term airway remodeling and exacerbation frequency 4, 5
Clinical Impact on Disease Outcomes
The triple combination significantly reduces moderate-to-severe COPD exacerbations, improves lung function (FEV1), and enhances quality of life compared to dual therapy 6, 7. Real-world evidence demonstrates:
- 12% lower rate of moderate-severe exacerbations compared to other triple therapy options 7
- Clinically meaningful improvement in CAT scores (mean reduction of 6.81 points) 8
- Improved trough FEV1 by approximately 0.08-0.09 L compared to ICS/LABA combinations 9
- 11% lower risk of all-cause mortality at 12 months compared to alternative triple therapies 7
Practical Considerations
Once-daily administration: All three components are formulated for single daily dosing via the Ellipta dry powder inhaler, which improves adherence compared to multiple-inhaler regimens 5, 6.
Common adverse effects to monitor:
- Oropharyngeal candidiasis (from ICS component) - advise mouth rinsing after use 1
- Increased pneumonia risk in COPD patients (ICS-related) 3, 1
- Cardiovascular effects (from LABA component) - use caution in patients with cardiovascular disorders 1
- Urinary retention (from LAMA component) - caution in prostatic hyperplasia 1
- Narrow-angle glaucoma worsening (from LAMA component) 1
Not for acute relief: Trelegy is maintenance therapy only and should not be used for acute bronchospasm or as rescue medication 1.
