UK Cholesterol Targets
In the UK, the target is total cholesterol <5.0 mmol/L and LDL cholesterol <3.0 mmol/L for patients with established cardiovascular disease or those at high risk (>15% 10-year CHD risk). 1
Risk-Stratified Targets
The Joint British Societies guidelines establish clear cholesterol thresholds based on cardiovascular risk status:
For Patients with Established CHD or Atherosclerotic Disease
- Total cholesterol: <5.0 mmol/L
- LDL cholesterol: <3.0 mmol/L
- All patients should have cholesterol reduced to consistently below these levels 1
For High-Risk Individuals Without Overt Disease
High risk is defined as absolute CHD risk >15% over 10 years:
- Total cholesterol: <5.0 mmol/L
- LDL cholesterol: <3.0 mmol/L
- When total cholesterol >5.0 mmol/L and CHD risk >15%: provide lifestyle advice and drug treatment if levels remain elevated on repeat measurements 1
Special Population: Diabetes Mellitus
Patients with diabetes require the same stringent targets:
- Total cholesterol: <5.0 mmol/L
- LDL cholesterol: <3.0 mmol/L
- These patients are considered at particularly high risk of CHD 1
Treatment Approach
When cholesterol exceeds targets:
- Familial hypercholesterolaemia or inherited dyslipidaemia: immediate lifestyle advice and drug treatment
- Total cholesterol >5.0 mmol/L with CHD risk >15%: lifestyle advice and drug treatment if sustained on repeat measurements
- CHD risk <15%: lifestyle advice with annual reassessment if risk approaches 15% 1
Important Clinical Context
Common pitfall: Real-world data shows that approximately 73% of British patients do not achieve these targets without medication changes or dose adjustments 2. Even among treated patients, significant gaps exist—only 27.1% achieved the total cholesterol target in one analysis 2.
Residual risk consideration: Among patients achieving the total cholesterol target of <5.0 mmol/L, approximately 60% still have residual dyslipidaemias (low HDL-C or high triglycerides), particularly those with diabetes 3. This means achieving the total cholesterol target alone may not eliminate cardiovascular risk.
The UK targets of <5.0 mmol/L for total cholesterol and <3.0 mmol/L for LDL-C are less stringent than newer European guidelines (which recommend LDL-C <1.4 mmol/L or <55 mg/dL for very high risk) 4, but remain the established UK standard based on the Joint British Societies recommendations 1.
