Sertraline Dosing Guidelines
Start sertraline at 50 mg once daily for adults with major depressive disorder and OCD, but use 25 mg once daily for panic disorder, PTSD, and social anxiety disorder, then increase to 50 mg after one week. 1
Adult Dosing
Standard Indications
For Major Depressive Disorder and Obsessive-Compulsive Disorder:
- Initial dose: 50 mg once daily 1
- Therapeutic range: 50-200 mg/day 2, 1
- Dose adjustments: Increase in 50 mg increments at intervals of at least 1 week 1
- Maximum dose: 200 mg/day 1
For Panic Disorder, PTSD, and Social Anxiety Disorder:
- Initial dose: 25 mg once daily for the first week 1
- After week 1: Increase to 50 mg once daily 1
- Therapeutic range: 50-200 mg/day 1
For Premenstrual Dysphoric Disorder:
- Initial dose: 50 mg/day (either continuous or luteal phase only) 1
- Dose range: 50-150 mg/day for continuous dosing; 50-100 mg/day for luteal phase dosing 1
- When using luteal phase dosing at 100 mg/day, use a 50 mg/day titration step for 3 days at the beginning of each luteal phase 1
Timing
- Administer once daily, either morning or evening 1
- The 24-hour elimination half-life supports once-daily dosing 1
Pediatric Dosing (Children and Adolescents)
For Obsessive-Compulsive Disorder (the only FDA-approved pediatric indication):
Children (ages 6-12):
- Initial dose: 25 mg once daily 1
- Therapeutic range: 25-200 mg/day 1
- Dose adjustments: Increase gradually, considering lower body weight to avoid excess dosing 1
Adolescents (ages 13-17):
Critical pediatric considerations:
- Only fluoxetine is FDA-approved for pediatric depression; escitalopram is approved only for adolescents ≥12 years 3
- Start at lower doses than adults to avoid behavioral activation and suicide-related events 3
- Dose changes should occur at intervals of at least 1 week 1
- Monitor weight regularly, as approximately 7% of children may experience >7% body weight loss 1
Elderly Patients
Use the same starting dose as younger adults (50 mg/day), but recognize that elderly patients have approximately 40% lower plasma clearance. 1
- Initial dose: 50 mg once daily 2
- No automatic dose reduction required based solely on age 1, 4
- Therapeutic range: 50-200 mg/day (same as adults) 2
- Mean effective dose in maintenance studies: 70 mg/day 1
Important considerations for elderly:
- Sertraline is preferred over paroxetine (more anticholinergic) and fluoxetine (greater agitation risk) in older adults 2
- Lower potential for drug interactions compared to other SSRIs makes it particularly suitable for elderly patients on multiple medications 4
- Steady-state achieved after 2-3 weeks (versus 1 week in younger patients) due to reduced clearance 1
Hepatic Impairment
In patients with hepatic impairment, use a lower or less frequent dose. 1
- Patients with mild hepatic impairment (Child-Pugh 5-8) show approximately 3-fold greater sertraline exposure 1
- Desmethylsertraline exposure is approximately 2-fold greater 1
- Specific recommendation: Reduce dose or frequency, though exact dosing not specified in FDA labeling 1
- Moderate to severe hepatic impairment has not been studied 1
- Approach with caution and use clinical judgment for dose titration 2, 1
Renal Impairment
No dose adjustment is necessary for renal impairment, including patients on hemodialysis. 1
- Sertraline pharmacokinetics are unaffected by mild to severe renal impairment 1
- Extensive hepatic metabolism means renal excretion is a minor elimination route 1
- Hemodialysis does not remove sertraline; no post-dialysis supplementation needed 5
- Elimination half-life may be prolonged (42-92 hours versus 24-36 hours normally) in end-stage renal disease, but clinical significance is minimal 5
Maintenance Treatment Duration
Continue treatment for 4-12 months after first episode of major depression; longer for recurrent depression. 2
- First episode: 4-12 months minimum after achieving response 2, 1
- Recurrent depression: Consider prolonged or indefinite treatment 2
- PTSD: Maintain for at least 28 weeks after initial 24-week response 1
- OCD and Panic Disorder: Several months or longer of sustained therapy 1
- Social Anxiety Disorder: Maintain for at least 24 weeks after initial 20-week response 1
Critical Warnings
- Always taper slowly when discontinuing to avoid withdrawal effects 3
- Monitor closely for behavioral activation, suicidality, and unusual behavior changes, especially in the first few months and after dose changes 3, 1
- Starting at higher than recommended doses increases risk of deliberate self-harm and suicide-related events 3
- Contraindicated with MAOIs 3
