Fluticasone 220 mcg Dosing
For asthma in adults and children ≥12 years using the 220 mcg/puff HFA MDI formulation, start with 1 puff twice daily (440 mcg/day total) for medium-dose therapy, which can be reduced to 1 puff once daily (220 mcg/day) for low-dose maintenance once control is achieved. 1
Asthma Dosing by Age and Severity
Adults and Children ≥12 Years (220 mcg/puff HFA MDI)
Low-dose range: 88-264 mcg/day total
- Typically 1 puff of 110 mcg twice daily OR 1-2 puffs of 44 mcg formulation twice daily
Medium-dose range: >264 mcg/day total
- 1 puff of 220 mcg twice daily = 440 mcg/day (standard medium dose)
- This falls within the >264 mcg threshold for medium-intensity therapy
High-dose range: >440 mcg/day total
- 2 puffs of 220 mcg twice daily = 880 mcg/day
- Exceeds the >440 mcg threshold for high-intensity therapy 1
Children 5-11 Years (220 mcg/puff HFA MDI)
Low-dose: 88-176 mcg/day total
- Use lower strength formulations (44 or 110 mcg/puff)
Medium-dose: >176-352 mcg/day total
- 1 puff of 220 mcg once daily = 220 mcg/day (within medium range)
High-dose: >352 mcg/day total
- 1 puff of 220 mcg twice daily = 440 mcg/day 1
Children 0-4 Years
The 220 mcg/puff strength is not appropriate for this age group. Use budesonide nebulizer suspension (the only FDA-approved ICS for children <4 years) or fluticasone HFA 44 mcg/puff with face mask 1
Allergic Rhinitis Dosing
Fluticasone propionate nasal spray is available as 50 mcg/spray, NOT 220 mcg/spray. The 220 mcg strength is exclusively for inhaled asthma therapy via HFA MDI, not for intranasal use 2, 3
For allergic rhinitis, use fluticasone propionate 50 mcg/spray:
- Adults: 2 sprays per nostril once daily (200 mcg total) OR 1 spray per nostril twice daily (200 mcg total) 2, 3
- Children ≥4 years: 1 spray per nostril once daily (100 mcg total), may increase to 2 sprays per nostril once daily (200 mcg total) if inadequate response 2, 3
COPD
Fluticasone is NOT FDA-approved as monotherapy for COPD. It is only indicated for COPD when combined with long-acting beta-agonists (e.g., fluticasone/salmeterol or fluticasone/vilanterol combinations). The 220 mcg/puff strength is not used in standard COPD combination products.
Key Clinical Considerations
Dosing Frequency
Twice-daily dosing is superior to once-daily dosing for the same total daily dose. Meta-analysis demonstrates that fluticasone given twice daily provides significantly greater improvements in FEV₁ (0.11 L difference), peak flow (12.9 L/min difference), and fewer withdrawals due to asthma exacerbations (OR 0.44) compared to once-daily administration 4. Divide the daily dose into two administrations for optimal asthma control 1, 4, 5
Dose-Response Relationship
The dose achieving 80-90% of maximum benefit is approximately 200-250 mcg/day total (considered "standard dose" in recent literature, though guidelines classify this as "low dose") 6. Starting with 1 puff of 220 mcg twice daily (440 mcg/day) provides medium-intensity therapy appropriate for moderate persistent asthma 1
Titration Strategy
- Start at medium dose (440 mcg/day with 220 mcg/puff twice daily) for moderate persistent asthma
- Step down to low dose (220 mcg/day = 1 puff once daily) after achieving control for 3+ months 1
- Monitor response every 4-6 weeks; discontinue if no clear benefit and adherence/technique are adequate 1
Safety Monitoring
- Growth velocity: Monitor height in children on medium-high doses, though effects may be transient 1
- Adrenal suppression: Risk increases with doses >500 mcg/day, especially with CYP3A4 inhibitors (ritonavir, ketoconazole) 1
- Local effects: Use spacer/valved holding chamber; rinse mouth after inhalation to reduce oral thrush, dysphonia 1
Common Pitfalls
- Do not confuse inhaled (mcg/puff) with intranasal (mcg/spray) formulations - they are completely different products
- Do not exceed 2 puffs per nostril daily (200 mcg) for nasal spray or 880 mcg/day for inhaled therapy without specialist consultation 1, 3
- Preparations are NOT interchangeable on a mcg-per-puff basis between different ICS products 1
