Kremizin (AST-120) Dosing for Chronic Kidney Disease
The recommended dosage of Kremizin (AST-120) for adult patients with chronic kidney disease is 6 grams per day, divided into three doses of 2 grams each (taken three times daily with meals).
Evidence-Based Dosing Recommendations
The optimal dosing of AST-120 has been established through multiple dose-ranging studies:
Standard Dosing Regimen
- Total daily dose: 6 grams/day
- Administration schedule: 2 grams three times daily (with breakfast, lunch, and dinner)
- Duration: Long-term continuous therapy until dialysis initiation or as clinically indicated
This 6 g/day dosing regimen demonstrated superior efficacy compared to lower doses in clinical trials 1, 2.
Dose-Response Evidence
The dose-ranging study by Schulman et al. 1 evaluated three different doses (0.9 g, 2.1 g, and 3.0 g three times daily) and concluded that 3 g three times daily (9 g total) is optimal for the US population, though this appears to be a dosing error in their conclusion as they tested 3 g TID which equals 9 g/day. However, the more widely validated and studied dose across multiple trials is 6 g/day in divided doses 2, 3.
Nakao et al. 2 specifically demonstrated that 6.0 g/day significantly slowed CKD progression (made the slope of reciprocal serum creatinine versus time less steep), while lower doses (<3.0 g/day and 3.0 g/day) did not show significant benefit. This establishes 6 g/day as the minimum effective dose.
Clinical Application Algorithm
When to initiate:
- CKD stages 3-5 (eGFR <60 mL/min/1.73 m²)
- Serum creatinine 1.2-3.0 mg/dL or higher
- Progressive deterioration despite standard therapy (RAS inhibitors, dietary protein restriction)
Dosing considerations:
- Start at full dose of 6 g/day divided TID
- No dose adjustment needed for renal or hepatic insufficiency based on pharmacokinetic data 4
- Take with meals to optimize gastrointestinal adsorption of uremic toxins
- Continue long-term (12+ months) for maximal benefit 3, 5
Important Clinical Caveats
Monitoring requirements (per KDIGO 2024 guidelines 6):
- Monitor eGFR and serum creatinine regularly (AST-120 does not interfere with creatinine measurements 7)
- Check electrolytes periodically
- AST-120 does not require the intensive monitoring needed for medications with narrow therapeutic windows
Drug interactions:
- AST-120 is an oral adsorbent that may bind other medications in the GI tract
- Administer other oral medications at least 2 hours before or after AST-120 to prevent adsorption
- Particularly important for medications with narrow therapeutic windows
Combination therapy:
- AST-120 should be added to standard CKD management, not used as monotherapy
- Continue RAS inhibitors (ACE-I/ARB), SGLT2 inhibitors, and dietary protein restriction as per KDIGO guidelines 6
- The combination of AST-120 with low-protein diet and RAS blockers showed additive benefits 3
Real-World Effectiveness
Recent real-world evidence supports the 6 g/day dosing:
- Propensity-matched cohort studies show AST-120 users had significantly lower risk of RRT initiation (HR 0.41) and mortality (HR 0.508) 5, 8
- Benefits were most pronounced in patients with early CKD (stage 3) and those with rapid progression 3
- Long-term use (>12 months) appears necessary for clinically meaningful outcomes 5, 9
Note: One large Korean RCT 10 showed no benefit, but this may reflect issues with adherence, study design, or population differences. The preponderance of evidence, including multiple Japanese studies and recent real-world data, supports efficacy at 6 g/day dosing.
