When to Give Ursodeoxycholic Acid (UDCA)

Ursodeoxycholic acid should be given to patients with primary biliary cholangitis (PBC) at a dose of 13-15 mg/kg/day as first-line therapy, but should NOT be used in patients with primary sclerosing cholangitis (PSC). 1

Primary Indications for UDCA

Primary Biliary Cholangitis (PBC) - STRONGLY RECOMMENDED

  • All patients with PBC should receive UDCA 13-15 mg/kg/day as the treatment of choice 1
  • Start immediately upon diagnosis in patients with:
    • Elevated alkaline phosphatase (AP)
    • Positive anti-mitochondrial antibodies (AMA ≥1:40) or AMA-M2
    • Even if asymptomatic
  • Greatest benefit occurs when started in early disease (Stage I-II) before progression to advanced fibrosis or cirrhosis 1
  • UDCA improves:
    • Serum bilirubin, alkaline phosphatase, gamma-glutamyl transferase
    • Histological features and delays disease progression
    • Reduces likelihood of liver transplantation or death when started early 1

Gallstone Dissolution - FDA APPROVED

UDCA is indicated for 2:

  • Radiolucent, noncalcified gallbladder stones <20 mm in greatest diameter
  • Patients who are poor surgical candidates due to:
    • Systemic disease
    • Advanced age
    • High anesthetic risk
    • Patient refusal of surgery
  • Dose: 8-10 mg/kg/day for up to 24 months
  • Expected dissolution rate: ~30% in unselected patients; up to 81% for stones ≤5 mm 2

Gallstone Prevention After Bariatric Surgery - RECOMMENDED

  • Give UDCA 250-600 mg daily for 6 months after laparoscopic sleeve gastrectomy or gastric bypass 3, 4, 5
  • Significantly reduces:
    • Cholecystectomy rates (8.3% vs 13.9%)
    • ERCP requirements (0.3% vs 2.5%)
    • Overall gallstone disease requiring intervention (8.9% vs 15.8%) 3
  • Number needed to treat to prevent one cholecystectomy: approximately 10 5
  • This aligns with American Society for Metabolic and Bariatric Surgery 2019 guidelines 3

Intrahepatic Cholestasis of Pregnancy (ICP) - CONDITIONALLY RECOMMENDED

  • UDCA 10-15 mg/kg/day (typically 300 mg 2-3 times daily or 500 mg twice daily) for bile acid levels ≥10 μmol/L 6
  • Primary benefit: maternal pruritus relief (usually within 1-2 weeks)
  • Perinatal outcome benefit is uncertain based on recent large RCT, but may reduce preterm births 6
  • Can titrate to maximum 21 mg/kg/day if pruritus persists
  • Well-tolerated; mild nausea/dizziness in up to 25% of patients 6

When NOT to Give UDCA

Primary Sclerosing Cholangitis (PSC) - STRONGLY CONTRAINDICATED

  • Do NOT use UDCA as medical therapy for adult PSC patients 7
  • Evidence shows:
    • No improvement in clinical outcomes (death, transplantation, histologic progression) 7
    • High-dose UDCA (28-30 mg/kg/day) may be HARMFUL - associated with increased risk of death, liver transplantation, and variceal development 1, 8
    • Low-to-moderate doses improve biochemistry but not mortality or morbidity 8
  • Exception: Consider in PSC/autoimmune hepatitis overlap syndrome, but use corticosteroids as primary therapy 7

Other Contraindications

  • Calcified gallstones (UDCA ineffective) 2
  • Gallstones >20 mm (poor dissolution rates) 2
  • Complete biliary obstruction
  • Acute cholecystitis or cholangitis

Dosing Summary by Indication

Indication Dose Duration Strength of Recommendation
PBC 13-15 mg/kg/day Indefinite STRONG [1]
PSC DO NOT USE N/A STRONG AGAINST [7,8]
Gallstone dissolution 8-10 mg/kg/day Up to 24 months Moderate [2]
Post-bariatric surgery 250-600 mg/day 6 months Strong [3,4,5]
ICP 10-15 mg/kg/day Until delivery Moderate [6]

Critical Pitfalls to Avoid

  1. Do not use UDCA in PSC - this is the most important contraindication, as high doses may worsen outcomes 8
  2. Do not delay UDCA in PBC - early treatment (Stage I-II) provides maximum benefit for mortality/transplantation reduction 1
  3. Do not use UDCA for calcified or large (>20 mm) gallstones - dissolution rates are negligible 2
  4. Monitor biochemical response in PBC at 12 months - inadequate response should prompt consideration of second-line agents 9, 10
  5. Cost and compliance barriers - 45% of patients cite cost as barrier to adherence after bariatric surgery 3
