Preferred Medications for COVID-19
For non-hospitalized patients with mild-to-moderate COVID-19 at high risk for severe disease, nirmatrelvir/ritonavir (Paxlovid) is the first-line treatment, with molnupiravir and remdesivir as alternatives when nirmatrelvir/ritonavir is contraindicated. 1
Treatment Algorithm by Risk Level
High-Risk Patients (Strong Recommendation)
Nirmatrelvir/ritonavir is the preferred agent based on high certainty evidence showing important reductions in hospitalization and moderate certainty evidence of survival benefit without increased adverse events 1. The WHO guideline panel concluded this represents a superior choice over other antivirals when available and when drug interactions are manageable 1.
Dosing and timing:
- 300 mg nirmatrelvir/100 mg ritonavir orally every 12 hours for 5 days
- Must be started within 5 days of symptom onset (as early as possible)
- Dose reduction to 150 mg/100 mg every 12 hours if eGFR 30-59 mL/min 1
Alternative agents for high-risk patients:
Molnupiravir (conditional recommendation): Use when nirmatrelvir/ritonavir is unavailable or contraindicated. Provides moderate certainty evidence of reduced hospitalization and mortality, but has significant safety concerns 1
- Dose: 800 mg orally every 12 hours for 5 days
- Critical contraindications: pregnancy, breastfeeding, children (due to mutagenic effects on bone growth), men planning conception within 3 months 1
Remdesivir: Consider when oral agents are contraindicated, particularly for pregnant patients and children where molnupiravir cannot be used 1
Moderate-Risk Patients (Conditional Recommendation)
Suggest using nirmatrelvir/ritonavir - provides high certainty evidence of important hospitalization reduction, though smaller absolute benefit than in high-risk patients 1. Many patients will value this benefit, but a minority may not.
Suggest against molnupiravir - the small benefits are likely outweighed by toxicity concerns for most patients in this risk category 1
Low-Risk Patients (Conditional/Strong Recommendation Against)
Suggest against nirmatrelvir/ritonavir - any benefits are trivial (high certainty) 1
Recommend against molnupiravir (strong recommendation) - trivial benefits do not justify toxicity risks 1
Critical Drug Interaction Considerations
Ritonavir is a major CYP450 inhibitor causing extensive drug-drug interactions during treatment and for several days after completion 1. This is a primary reason to choose alternative agents. Use the Liverpool COVID-19 drug interaction tool to check compatibility 1. Approximately 40% of physicians report that drug interactions deter them from prescribing nirmatrelvir/ritonavir 2.
Special Populations
Pregnant/breastfeeding patients:
- Nirmatrelvir/ritonavir is an option - no serious adverse reactions reported in WHO Vigibase to date 1
- Molnupiravir is contraindicated - causes embryo-fetal lethality and teratogenicity in animal studies 1
- Remdesivir may be preferred alternative 1
Children:
- Molnupiravir is contraindicated - causes growth plate abnormalities and decreased bone formation in animal studies 1
Immunocompromised/vaccinated patients:
- Same recommendations apply, though evidence in these subgroups remains uncertain 1
Common Pitfalls to Avoid
Timing failure: All antivirals must be started within 5 days of symptom onset - efficacy diminishes rapidly after this window 1, 3
Ignoring drug interactions: Failure to check ritonavir interactions can lead to serious adverse events or treatment failures 1, 2
Inappropriate molnupiravir use: Do not use in pregnancy, children, young adults (higher lifetime genotoxicity risk), or men planning conception 1
Combination therapy: No evidence supports combining antivirals - do not do this 1
Renal dosing: Adjust nirmatrelvir/ritonavir dose for eGFR 30-59 mL/min; use with caution in severe renal/hepatic impairment 1
Comparative Effectiveness
Indirect comparisons show nirmatrelvir/ritonavir probably reduces hospitalization more than molnupiravir (moderate certainty) and shows little or no difference compared to remdesivir (low certainty) 1. The superiority of nirmatrelvir/ritonavir is based on efficacy, molnupiravir's safety concerns, and remdesivir's parenteral administration burden 1.
