Trastuzumab Dosing for Metastatic Gastric Cancer
For HER2-positive metastatic gastric cancer, administer trastuzumab at an initial loading dose of 8 mg/kg IV over 90 minutes, followed by maintenance doses of 6 mg/kg IV every 3 weeks, in combination with chemotherapy (typically fluoropyrimidine plus platinum agent). 1
Standard Dosing Regimen
The FDA-approved dosing for metastatic HER2-overexpressing gastric cancer is 1:
- Loading dose: 8 mg/kg IV infusion over 90 minutes on day 1
- Maintenance dose: 6 mg/kg IV infusion over 30-90 minutes every 3 weeks
This differs from the breast cancer regimen and should not be substituted with weekly dosing schedules used in breast cancer.
Key Clinical Considerations
Infusion Duration: While the initial loading dose requires 90 minutes, maintenance infusions can be safely shortened to 30 minutes after the first dose is well-tolerated 2. A Korean multicenter study of 128 patients demonstrated that 30-minute maintenance infusions were well-tolerated with only 26% experiencing infusion-related reactions and no cardiac events, while maintaining efficacy (63% objective response rate).
Combination Chemotherapy: The standard backbone is fluoropyrimidine (capecitabine or 5-FU) plus cisplatin, based on the ToGA trial that established this regimen 1. However, real-world evidence suggests that capecitabine-based regimens (HXP) may offer superior outcomes compared to 5-FU-based regimens (HFP), with median overall survival of 13.27 months versus 9.89 months 3.
Important Caveats
Do Not Use Breast Cancer Dosing: The weekly schedule (4 mg/kg loading, then 2 mg/kg weekly) used in breast cancer is NOT the approved regimen for gastric cancer 1. Gastric cancer requires the higher every-3-week dosing schedule.
Monitoring Requirements: Unlike breast cancer where cardiac monitoring is specified at baseline, 3,6, and 9 months, specific cardiac monitoring intervals are not detailed in the gastric cancer FDA label, though cardiac assessment remains clinically prudent given trastuzumab's known cardiotoxicity profile.
Dose Adequacy Concerns: Case reports suggest some gastric cancer patients may require higher doses than standard to achieve therapeutic levels 4, 5, possibly due to higher tumor burden or increased drug clearance compared to breast cancer. However, the standard 8 mg/kg loading/6 mg/kg maintenance remains the evidence-based recommendation unless therapeutic drug monitoring indicates otherwise.
Evolving Treatment Landscape
While trastuzumab plus chemotherapy remains foundational, newer evidence supports adding pembrolizumab to trastuzumab-chemotherapy in PD-L1 positive disease (KEYNOTE-811 trial) 6. For second-line therapy after trastuzumab failure, trastuzumab deruxtecan has demonstrated superior overall survival (14.7 vs 11.4 months) compared to ramucirumab-paclitaxel 7.
The trastuzumab dose itself does not change when combined with immunotherapy—maintain the 8 mg/kg loading/6 mg/kg every 3 weeks schedule.
