Posaconazole Dosing in Adults
For prophylaxis of invasive fungal infections, administer posaconazole delayed-release tablets 300 mg twice daily on day 1, then 300 mg once daily starting day 2; for treatment of invasive fungal infections, use 300 mg twice daily on day 1, then 300 mg once daily, or consider higher doses (400 mg daily) for optimal therapeutic efficacy. 1
Dosing by Formulation
The dose of posaconazole varies significantly by formulation, and these formulations are NOT interchangeable 2:
Delayed-Release Tablets (Preferred Formulation)
- Prophylaxis: 300 mg twice daily on day 1 (loading), then 300 mg once daily 1
- Treatment: 300 mg twice daily on day 1 (loading), then 300 mg once daily 3
- Can be taken with or without food 1
- Swallow whole; do not crush, divide, or chew 1
Intravenous Solution
- Loading: 300 mg IV twice daily on day 1
- Maintenance: 300 mg IV once daily 2
- Preferred route when oral administration is not feasible 3
Oral Suspension (Less Preferred)
- Initial dosing: 200 mg (5 mL) four times daily
- After stabilization: 400 mg (10 mL) twice daily 2
- Must be taken with fatty food for optimal absorption 2
- This formulation has unpredictable bioavailability and is generally avoided when tablets or IV are available 4
Clinical Context-Specific Dosing
For Mucormycosis (COVID-Associated or Otherwise)
- Primary therapy alternative: 300 mg IV or PO twice daily on day 1, then 300 mg once daily 3
- Maintenance therapy: 300 mg IV or PO twice daily on day 1, then 300 mg once daily for 3-6 months 3
- IV formulation preferred over oral 3
For Invasive Aspergillosis
The 2023 Taiwan guidelines and 2019 global mucormycosis guidelines consistently recommend the same dosing as above 2, 3, 2. However, recent pharmacokinetic/pharmacodynamic modeling suggests that standard dosing may be insufficient for treatment 5. For treatment of aspergillosis (not prophylaxis), consider:
- Higher-dose regimen: 200 mg every 12 hours (delayed-release tablets) to achieve optimal therapeutic efficacy 5
- Standard 300 mg daily may only be adequate for prophylaxis or treatment of highly susceptible organisms (MIC ≤0.125 µg/mL) 5
Critical Considerations
Therapeutic Drug Monitoring (TDM)
TDM is strongly recommended for all patients receiving posaconazole for treatment, regardless of formulation 6. Despite manufacturer claims that delayed-release tablets achieve predictable levels:
Target concentrations:
Factors Reducing Posaconazole Levels
Monitor closely and consider dose adjustment in patients with:
- Body weight ≥90 kg or BMI ≥30 kg/m² 6
- Acid suppressive therapy (proton pump inhibitors, H2 blockers) 9
- Severe diarrhea or vomiting 1
- Concomitant corticosteroids (methylprednisolone ≥0.7 mg/kg/day) 9
Dose Adjustments Based on TDM
If trough concentrations are subtherapeutic with delayed-release tablets 6, 10:
- Consider increasing to 400 mg once daily for maintenance
- For oral suspension: increase frequency to 200 mg four times daily or dose to 300 mg three times daily 10
- Initial doses higher than 300 mg daily should be strongly considered for patients with risk factors 6
Special Populations
- Renal impairment: No dose adjustment needed for oral formulations 1
- Hepatic impairment: No specific dose adjustment recommended, but monitor closely
- Critically ill patients: Consider loading regimen of 300 mg q12h and 300 mg q8h in first two days for patients <100 kg and ≥100 kg respectively, then 400 mg daily maintenance 11
Duration of Therapy
- Prophylaxis: Continue until recovery from neutropenia or immunosuppression 1
- Treatment: Minimum 4-6 weeks for induction/consolidation, then 3-6 months maintenance until resolution of clinical signs and symptoms 3
Common Pitfalls
- Assuming formulations are interchangeable - they are not; dosing differs significantly
- Not performing TDM for treatment - even with delayed-release tablets, nearly half of patients have subtherapeutic levels
- Using standard 300 mg daily for treatment - may be insufficient; consider 400 mg daily or higher based on TDM
- Ignoring drug interactions - particularly with acid suppressants, which significantly reduce absorption
