When to Order Alpha-Fetoprotein (AFP) Tumor Marker
AFP should be ordered primarily for hepatocellular carcinoma (HCC) surveillance in at-risk patients with cirrhosis or advanced fibrosis, combined with ultrasound every 6 months, and for diagnosis/monitoring of suspected germ cell tumors in conjunction with hCG and LDH.
Primary Indications
Hepatocellular Carcinoma Surveillance and Diagnosis
The most appropriate use of AFP is for HCC surveillance in patients with cirrhosis or advanced fibrosis (F3-F4), where it should be combined with ultrasound rather than used alone 1. The combination strategy increases sensitivity for early-stage HCC detection from 51.6% to 74.1%, though specificity drops slightly from 87.9% to 83.9% 1.
Key points for HCC surveillance:
- Use AFP combined with ultrasound every 6 months in patients with cirrhosis
- AFP alone has poor sensitivity (49.1%) for early-stage HCC detection 1
- The combination misses over one-third of early HCCs but remains the best available strategy
- Consider a cutoff of 5 ng/mL in the current era (post-DAA), as baseline AFP levels have decreased 2
- Elevated AFP occurs in approximately 75% of HCC cases 3
Germ Cell Tumor Diagnosis and Monitoring
Order AFP alongside hCG and LDH when germ cell tumors are clinically suspected - not for screening asymptomatic adults 4. This includes:
- Patients with testicular masses or extragonadal masses suspicious for germ cell tumors
- Monitoring treatment response in diagnosed germ cell tumors
- Surveillance after treatment completion
Do not use AFP for screening asymptomatic adults for germ cell tumors - the low incidence makes this neither cost-effective nor mortality-reducing 4.
Important Caveats and False-Positive Causes
Benign Liver Conditions
AFP elevation is not specific for malignancy. Common benign causes include 4, 5:
- Acute and chronic hepatitis (viral or toxic)
- Cirrhosis and hepatic regeneration
- Biliary tract obstruction
- Alcohol abuse
Some individuals have constitutively elevated AFP (15-30 ng/mL chronically) - cancer-related AFP shows a consistent pattern of increasing values 4.
Other Malignancies
- Gastric cancer (though only 3% of cases, making routine testing not worthwhile) 7
- Pancreatic cancer
- Lung cancer
- Colon cancer
When evaluating poorly differentiated cancers, elevated AFP does not definitively diagnose germ cell tumors or HCC - other malignancies must be considered 4.
Physiological Elevations
- Newborns (normal developmental finding)
- Pregnancy
Monitoring Treatment Response
AFP is valuable for monitoring HCC treatment response - serial measurements often detect recurrence more effectively than imaging 3, 8. However, be aware that:
- Tumor lysis during the first week of chemotherapy can cause transient AFP elevation 4
- If AFP rises between day 1 of cycle 1 and day 1 of cycle 2, repeat midway through cycle 2 to confirm whether levels are declining 4
Enhanced Strategies
For improved HCC detection accuracy, consider combining AFP with:
- AFP-L3 (lectin-bound AFP) and DCP (des-gamma-carboxy prothrombin) 1, 9
- The GALAD score (combines AFP, AFP-L3, DCP, age, and gender) shows promise 1
- Use DCP cutoff of 40 mAU/mL alongside AFP 5 ng/mL 2
What NOT to Do
- Do not order AFP for routine cancer screening in asymptomatic patients without risk factors
- Do not use AFP alone for HCC surveillance - always combine with imaging 1
- Do not interpret mildly elevated AFP (15-30 ng/mL) as definitive evidence of malignancy without considering chronic liver disease and constitutional elevation 4
- Do not order AFP routinely for gastric cancer staging - CEA and CA19-9 are superior 7
