Management of HELLP Syndrome

Women with suspected HELLP syndrome require prompt delivery once maternal coagulopathy and severe hypertension have been corrected, as delayed delivery worsens maternal outcomes. 1

Initial Assessment and Diagnosis

HELLP syndrome should be recognized as a manifestation of severe preeclampsia 1. The diagnosis requires:

  • Hemolysis (elevated LDH >600 U/L, peripheral smear examination, low haptoglobin)
  • Elevated liver enzymes (AST ≥70 U/L)
  • Low platelets (<100 × 10⁹/L)

Maternal assessment must include blood pressure monitoring, proteinuria evaluation, and biochemical testing. Liver function tests are frequently associated with adverse maternal outcomes but should not guide care in isolation 1.

Blood Pressure Management

Non-Severe Hypertension (140-159/90-109 mmHg)

Initiate oral antihypertensive therapy with labetalol, nifedipine, or methyldopa 1.

Severe Hypertension (≥160/110 mmHg)

Urgent treatment in a monitored setting is mandatory. Use oral labetalol, nifedipine, or methyldopa as first-line agents. Intravenous labetalol or hydralazine may be required for refractory cases 1. Target blood pressure should remain below 155/105 mmHg 2.

Seizure Prophylaxis

Magnesium sulfate must be administered to women with HELLP syndrome and co-existing severe hypertension to prevent eclamptic seizures 1. Additionally, use magnesium sulfate as a neuroprotective agent if delivery is required before 32 weeks' gestation. Dosing should follow local/national protocols 1.

Platelet Management

Consider platelet transfusion when platelet count falls below 100 × 10⁹/L, as this threshold is associated with increased risk of abnormal coagulation and adverse maternal outcomes 1. Be particularly vigilant when platelets drop below 20 × 10⁹/L, as this markedly increases the risk of hepatic hemorrhage or hematoma 1.

Corticosteroid Use

For Maternal Outcomes

Do NOT give corticosteroids to improve maternal outcomes in HELLP syndrome 1. This is a strong recommendation based on level 1 evidence showing no maternal benefit.

For Fetal Lung Maturity

Administer high-dose dexamethasone or betamethasone for fetal lung maturation if delivery will occur before 35 weeks' gestation 1. Use either:

  • Betamethasone 12 mg × 2 doses, 24 hours apart, OR
  • Dexamethasone 6 mg × 4 doses, 12 hours apart

A single course is recommended between 24-34 weeks 2. Avoid repeated doses due to potential long-term adverse effects on the fetal brain 2.

Timing and Mode of Delivery

Delivery Timing Algorithm

After 34 weeks' gestation: Deliver promptly once coagulopathy and severe hypertension are corrected 1.

Before 34 weeks' gestation:

  • If maternal condition deteriorates or fetal distress develops → immediate delivery 2
  • In highly selected cases with stable maternal-fetal status → conservative management for up to 48 hours may be considered, though this increases maternal risk 1
  • Before 32 weeks with very early HELLP: Expectant management is controversial and carries increased maternal risk. Only consider in absence of severe symptoms with intensive monitoring 1

Mode of Delivery

Vaginal delivery is preferable when feasible 1, 3. Mode of delivery depends on gestational age, fetal condition, maternal disease severity, and cervical status 1. If the cervix is unfavorable, cervical ripening followed by labor induction is reasonable 2.

Monitoring for Complications

Hepatic Complications

Perform abdominal ultrasound if symptoms suggest hepatic hematoma, including abdominal pain, epigastric pain, or right shoulder pain 1. Hepatic hemorrhage risk is substantially elevated with platelet counts <20 × 10⁹/L 1.

Hepatic Failure

If signs of hepatic failure develop that may require transplantation, make early referral to a transplant center 1. This is critical for maternal survival in severe cases.

Postpartum Care

Continue close surveillance for at least 48 hours after delivery 3, 2. Maintain strict fluid balance to avoid pulmonary edema. Magnesium sulfate is typically continued for 24 hours postpartum in cases with severe hypertension 1. General anesthesia can precipitate uncontrolled hypertension, requiring multidisciplinary team involvement including obstetric anesthesia 1.

Critical Pitfalls to Avoid

  • Do not delay delivery once diagnosis is established at ≥34 weeks or if maternal/fetal condition deteriorates at any gestational age
  • Do not use corticosteroids for maternal treatment (only for fetal lung maturity)
  • Do not miss hepatic complications - maintain high suspicion with abdominal pain
  • Do not provide regional anesthesia (spinal/epidural) with severe thrombocytopenia or coagulopathy - general anesthesia may be the only safe option 1
  • Do not underestimate postpartum risk - approximately 30% of cases develop within 48 hours after delivery 3, 2
