IV Digoxin Loading Dose
For adults requiring IV digoxin loading, administer a total loading dose of 8-12 mcg/kg based on ideal body weight, given as half the total dose initially, then one-quarter of the loading dose every 6-8 hours twice 1.
Loading Dose Administration
The FDA-approved loading regimen is structured as follows 1:
- Total loading dose: 8-12 mcg/kg (based on ideal body weight for adults and pediatric patients over 10 years)
- Initial dose: Give 50% of total loading dose
- Subsequent doses: Give 25% of total loading dose at 6-8 hour intervals (twice)
- Administration rate: Infuse over at least 5 minutes to prevent vasoconstriction
- Maximum single injection site: 500 mcg for adults
Dilution Guidelines
- Can be given undiluted or diluted with ≥4-fold volume of sterile water, 0.9% NaCl, or 5% dextrose
- Use diluted product immediately
- Do not mix with other drugs in same container or IV line 1
Critical Dosing Adjustments
Reduce loading doses to 6-10 mcg/kg for patients with creatinine clearance <60 mL/min 2. Evidence demonstrates that patients with impaired renal function are 2.6 times more likely to experience toxic digoxin concentrations with standard loading doses 2.
High-Risk Populations Requiring Dose Reduction:
- Age >70 years: Use lower end of dosing range
- Weight <60 kg: Calculate based on lean body weight
- CrCl <50 mL/min: Reduce to 6-10 mcg/kg total loading dose
- Obesity or edema: Dose based on lean body weight, not total body weight 1
Maintenance Dosing After Loading
Following the loading regimen, initiate maintenance dosing at 2.4-3.6 mcg/kg/day (given once daily) for adults with normal renal function 1.
For practical maintenance dosing 1:
- CrCl ≥60 mL/min: 125-250 mcg daily
- CrCl 30-59 mL/min: 125 mcg daily
- CrCl 15-29 mL/min: 125 mcg every other day
- CrCl <15 mL/min: 62.5-125 mcg every other day
The maintenance dose can be calculated using: Total Maintenance Dose = Loading Dose × % Daily Loss/100, where % Daily Loss = 14 + (CrCl/5) 1.
Safety Considerations
Avoid loading doses exceeding 12 mcg/kg based on ideal body weight, as this significantly increases the risk of supratherapeutic levels (≥1.2 ng/mL) without improving rate control efficacy 3. Recent data shows that doses >12 mcg/kg result in 50.6% of patients achieving levels ≥1.2 ng/mL compared to only 30% with doses ≤12 mcg/kg 3.
Common Pitfalls:
- Using total body weight instead of ideal body weight in obese patients leads to overdosing
- Failing to adjust for renal dysfunction is the most common cause of toxicity
- Concomitant amiodarone increases digoxin levels substantially—reduce digoxin dose by 50% 4
- Bolus administration can cause coronary vasoconstriction—always infuse over ≥5 minutes 1
Target Serum Concentrations
While the 2022 ACC/AHA/HFSA guidelines note that digoxin has limited mortality benefit in heart failure 5, when used for rate control or symptom management, target serum digoxin concentrations of 0.5-0.8 ng/mL for heart failure 4 or 0.8-1.5 ng/mL for acute rate control of atrial arrhythmias 6. Levels ≥1.2 ng/mL are associated with increased mortality risk 3.
Monitoring Timing:
- Draw first level 6-24 hours after completing loading dose
- Steady-state achieved in 7-19 days depending on renal function (longer with worse renal function) 1
Clinical Context
The 2022 heart failure guidelines assign digoxin only a Class 2b recommendation (may be considered) for reducing hospitalizations in symptomatic HFrEF despite GDMT, with no mortality benefit demonstrated 5. Loading doses are primarily indicated when rapid rate control is urgently needed and oral administration is not feasible 1.
