Latuda Dosing for Bipolar Disorder
For bipolar I depression in adults, start Latuda at 20 mg once daily with food (at least 350 calories), with an effective dose range of 20-120 mg/day and a maximum dose of 120 mg/day. 1
Adult Dosing for Bipolar Depression
The FDA-approved dosing for depressive episodes associated with bipolar I disorder is straightforward:
- Starting dose: 20 mg once daily
- Effective range: 20-120 mg/day
- Maximum dose: 120 mg/day
- No initial titration required 1
Critical administration requirement: Latuda must be taken with food containing at least 350 calories. Food increases absorption approximately 2-fold (AUC) and 3-fold (Cmax), and all clinical trials administered the medication with food 1. Without food, therapeutic levels may not be achieved.
Monotherapy vs. Adjunctive Therapy
The same dosing applies whether using Latuda as:
- Monotherapy (20-120 mg/day)
- Adjunctive therapy with lithium or valproate (20-120 mg/day) 1
Important caveat: In the monotherapy study, the higher dose range (80-120 mg/day) did not provide additional efficacy compared to the lower dose range (20-60 mg/day) 1. Recent meta-analysis confirms that 40-60 mg daily represents the optimal therapeutic dose, with 50 mg showing the best balance of efficacy (effect size -0.60 for depression) and tolerability 2. Higher doses (>60 mg) increase side effects without proportional benefit and raise blood glucose levels above 70 mg 2.
Pediatric Dosing (Ages 10-17)
For children and adolescents with bipolar depression:
- Starting dose: 20 mg once daily
- Effective range: 20-80 mg/day
- Maximum dose: 80 mg/day
- Titration: May increase after one week based on clinical response
- Clinical reality: Most patients (67%) in trials received 20-40 mg/day 1
Dose Adjustments for Special Populations
Renal Impairment
- Moderate impairment (CrCl 30-50 mL/min): Start 20 mg/day, max 80 mg/day
- Severe impairment (CrCl <30 mL/min): Start 20 mg/day, max 80 mg/day 1
Hepatic Impairment
- Moderate (Child-Pugh 7-9): Start 20 mg/day, max 80 mg/day
- Severe (Child-Pugh 10-15): Start 20 mg/day, max 40 mg/day 1
Drug Interactions
With moderate CYP3A4 inhibitors (diltiazem, erythromycin, fluconazole, verapamil):
- If adding inhibitor to existing Latuda: Reduce Latuda dose to half
- If starting Latuda with inhibitor present: Start 20 mg/day, max 80 mg/day
- Avoid grapefruit and grapefruit juice 1
Strong CYP3A4 inhibitors (ketoconazole, clarithromycin, ritonavir) and strong inducers (rifampin, carbamazepine, phenytoin, St. John's wort) are contraindicated 1.
Clinical Considerations
Efficacy Timeline
- Significant improvement typically seen by week 6
- Long-term use beyond 6 weeks has not been established in controlled studies; periodically reassess need for continued treatment 1
What Latuda Does NOT Treat
Latuda is NOT approved for acute mania. The efficacy for mania associated with bipolar disorder has not been established 1. The older guidelines [3-3] do not mention lurasidone because they predate its approval; they recommend lithium, valproate, and other atypical antipsychotics for acute mania.
Side Effect Profile
Common adverse events include nausea, akathisia, somnolence, and headache 4. Akathisia risk may exceed other modern antipsychotics 5. Metabolic effects are favorable: minimal weight gain at doses <60 mg (0.38 kg at 40 mg), but weight and glucose increase at higher doses 2. Prolactin elevation occurs in both males and females 2.
Risk of Mood Switch
While generally considered to have lower risk of inducing mania compared to conventional antidepressants 6, case reports document manic episodes following lurasidone initiation, particularly when mood stabilization is inadequate 7. Monitor closely for emerging manic symptoms, especially during the first 1-2 weeks of treatment.
Practical Dosing Algorithm
- Start 20 mg once daily with ≥350 calorie meal
- Assess response at 1-2 weeks
- If inadequate response and good tolerability, increase to 40-60 mg/day (optimal range per meta-analysis 2)
- If still inadequate response, may increase to max 120 mg/day, though evidence suggests diminishing returns above 60 mg
- Adjust for renal/hepatic impairment and drug interactions as outlined above
