Standard Starting Dose of Envarsus XR in De Novo Kidney Transplant
The recommended starting dose of Envarsus XR in de novo kidney transplant recipients receiving mycophenolate mofetil 500 mg twice daily and corticosteroids is 0.14 mg/kg once daily, administered within 48 hours of graft reperfusion 1.
Dosing Details
The FDA-approved starting dose is 0.14 mg/kg/day given once daily 1. This represents the official labeled dose for Envarsus XR in the de novo kidney transplant setting when used with:
- Mycophenolate mofetil (MMF) at approximately 1 gram twice daily
- Corticosteroids
- IL-2 receptor antagonist induction therapy (basiliximab in 96-97% of patients)
Important Dosing Considerations
Weight-based calculation: Use actual body weight for dosing. If the patient weighs more than 120% of ideal body weight, use adjusted body weight instead 2.
Target trough concentrations:
- First 30 days: 6-11 ng/mL
- After 30 days: 4-11 ng/mL 1
Clinical Trial Context
In the pivotal Study 1, investigators initially used 0.17 mg/kg once daily (approximately 1.2-fold higher than the currently recommended dose), but the FDA-approved label specifies 0.14 mg/kg/day as the standard starting dose 1. The higher dose used in the trial resulted in tacrolimus trough concentrations that were above target range during the first week post-transplant 1.
Critical Dosing Adjustments
CYP3A5 expressers require higher doses: Patients who are CYP3A5 intermediate or extensive metabolizers need approximately 0.19-0.20 mg/kg/day to achieve therapeutic levels, compared to 0.13 mg/kg/day for non-expressers 2. These patients take longer to reach therapeutic concentrations (13.5 days vs 4.5 days) and maintain lower trough levels throughout treatment.
Elderly patients (>60 years): Lower doses achieve similar blood concentrations with less variability compared to younger patients 3.
Monitoring Strategy
- Check tacrolimus trough levels on Days 2,3,4,7,10,14,21, and 30, then at regular intervals 1
- On Day 2, expect that only about one-third of patients will be within target range using standard dosing 1
- Dose adjustments should be made based on trough concentrations and clinical response
Common Pitfalls
Do not use a 1:1 conversion ratio if you're thinking about immediate-release tacrolimus equivalents. Envarsus XR has different pharmacokinetics and requires approximately 80% of the total daily dose of immediate-release tacrolimus when converting stable patients 4, but this is not applicable to de novo dosing.
Avoid interchanging formulations without appropriate dose modifications, as Prograf, Advagraf, and Envarsus XR are not bioequivalent 5.
