Start Qelbree at 100 mg once daily for children ages 6-11 years, 200 mg once daily for adolescents ages 12-17 years, and 200 mg once daily for adults, with weekly titration based on response and tolerability to maximum doses of 400 mg (children and adolescents) or 600 mg (adults). 1
Dosing by Age Group
Children (Ages 6-11 Years)
- Starting dose: 100 mg orally once daily
- Titration: Increase by 100 mg increments at weekly intervals
- Maximum dose: 400 mg once daily
- Adjust based on response and tolerability 1
Adolescents (Ages 12-17 Years)
- Starting dose: 200 mg orally once daily
- Titration: After 1 week, may increase by 200 mg increment
- Maximum dose: 400 mg once daily
- Adjust based on response and tolerability 1
Adults (Ages 18+ Years)
- Starting dose: 200 mg orally once daily
- Titration: Increase by 200 mg increments weekly
- Maximum dose: 600 mg once daily
- Adjust based on response and tolerability 1, 2
Administration Instructions
Administer once daily with or without food. 1 The capsules can be:
- Swallowed whole (do not cut, crush, or chew), OR
- Opened and entire contents sprinkled over 1 teaspoon or tablespoon of pudding or applesauce
- Consume entire mixture without chewing within 15 minutes (pudding) or 2 hours (applesauce)
- Do not store for future use 1
Special Population: Severe Renal Impairment
For patients with severe renal impairment (eGFR <30 mL/min/1.73m²):
- Starting dose: 100 mg once daily
- Titration: Increase by 50-100 mg increments weekly
- Maximum dose: 200 mg once daily
- No adjustment needed for mild-to-moderate renal impairment (eGFR 30-89 mL/min/1.73m²) 1
Pre-Treatment Requirements
Before initiating Qelbree, you must:
- Assess heart rate and blood pressure (repeat after dose increases and periodically during treatment) 1
- Screen for psychiatric history: personal or family history of suicide, bipolar disorder, and depression 1
Evidence for Efficacy and Optimal Dosing
The 2024 American Journal of Obstetrics and Gynecology guidelines note that Qelbree is approved in the United States for children and adolescents aged 6 and older, with the standard dosing regimen of starting at 200 mg once daily and titrating by 200 mg increments weekly to a maximum of 600 mg daily 3. However, note that these guidelines emphasize limited data on efficacy for adults and no safety data in pregnancy or breastfeeding 3.
Recent meta-analysis data from 2024 suggest that moderate doses (200-400 mg or 6-8 mg/kg) may provide optimal treatment outcomes, with a bell-shaped dose-response curve indicating doses above 400 mg or 7 mg/kg may not confer additional efficacy 4. The temporal trends analysis showed treatment effects plateau at approximately weeks 4-6, with the 200-400 mg dose ranges showing more gradual and sustained response curves 4.
Clinical trial data demonstrate statistically significant improvements in ADHD symptoms across all age groups, with effect sizes of approximately 0.33 in adolescents 5 and clinically meaningful improvements in adults 2. Long-term safety data from a phase 3 open-label extension trial (median exposure 260 days, up to 1896 days) showed median modal doses of 300 mg/day for children and 400 mg/day for adolescents, supporting these as practical target doses 6.
Critical Safety Warnings
Black Box Warning: Suicidal Thoughts and Behaviors
Monitor closely for clinical worsening and emergence of suicidal thoughts and behaviors. 1 In clinical trials:
- Pediatric patients: 0.9% (9/1019) on Qelbree vs 0.4% (2/463) on placebo reported suicidal ideation/behavior
- Adult patients: 1.6% (3/189) on Qelbree vs 0% on placebo reported suicidal ideation
- No completed suicides occurred in trials 1
Common Adverse Events (≥5% and twice placebo rate)
Pediatric patients: somnolence, decreased appetite, fatigue, nausea, vomiting, insomnia, irritability 1
Adult patients: insomnia, headache, somnolence, fatigue, nausea, decreased appetite, dry mouth, constipation 1
Advise patients about potential somnolence and fatigue when driving or operating machinery. 1
Contraindications
Qelbree is absolutely contraindicated with:
- Concomitant MAOI use or within 14 days of discontinuing an MAOI (risk of hypertensive crisis)
- Concomitant use with sensitive CYP1A2 substrates or those with narrow therapeutic range 1
Important Clinical Caveats
- Limited adult efficacy data: The 2024 guidelines note that data on efficacy, safety, and tolerability for adults remain limited 3
- Pregnancy/breastfeeding: No published safety data exist; alternative agents are preferred 3
- Long-term use: Periodically reevaluate the need for continued treatment and adjust dosage as needed 1
- Discontinuation rate: Adverse events led to discontinuation in 8.2% of pediatric patients in long-term studies 6 and 9.0% of adults 2
Monitoring During Treatment
- Heart rate and blood pressure at each dose increase and periodically thereafter 1
- Suicidality monitoring using validated scales (e.g., C-SSRS) 1
- Growth parameters in pediatric patients (though growth delays have not been prominently reported with viloxazine, unlike some other ADHD medications)
- Emergence of insomnia, irritability, or mood changes 1
