Important Safety Considerations
Regarding blurred vision specifically: Blurred vision is a recognized adverse effect of Remeron 2. The FDA label warns that mirtazapine can cause mild pupillary dilation, which may lead to angle-closure glaucoma in susceptible individuals 1. If you experience eye pain, vision changes, or swelling/redness around the eye, contact your healthcare provider immediately 1.
Critical warnings while taking Remeron:
- Avoid alcohol completely - it potentiates sedative effects 1
- Do not drive or operate machinery until you know how the medication affects you, as it causes significant drowsiness and impairs judgment 1
- Avoid benzodiazepines during treatment 1
What to Expect
Timing of effects:
- Sedation occurs immediately and is prominent 1
- Antidepressant effects may take several weeks
- Mirtazapine has faster onset of action compared to SSRIs (within 4 weeks) 3
Common side effects to monitor:
- Drowsiness (very common - 47% in studies) 4
- Weight gain (10% of patients) 4
- Increased appetite 1
- Dry mouth 2
Critical Pitfalls to Avoid
Never stop abruptly - discontinuation syndrome can occur. Discuss any tapering plan with your provider 1
Seek immediate medical attention if you develop:
- Fever, sore throat, or flu-like symptoms (low white blood cell count) 1
- Severe skin reactions with rash and fever 1
- Signs of serotonin syndrome: agitation, confusion, fast heart rate, high fever, muscle rigidity 1
The bedtime dosing strategy directly addresses the medication's sedative profile while maximizing therapeutic benefit for depression.
