Leflunomide Dosing and Titration
The standard leflunomide regimen consists of a loading dose of 100 mg daily for 3 days followed by a maintenance dose of 20 mg daily, though initiating with 20 mg daily without a loading dose is preferred in patients at higher risk for adverse events. 1, 2
Loading Dose Considerations
The loading dose accelerates achievement of steady-state concentrations due to leflunomide's long half-life (approximately 2 weeks) 3. However, eliminating the loading dose may decrease the risk of adverse events, particularly for patients at increased risk of hematologic or hepatic toxicity 2. This is especially important for:
- Patients receiving concomitant methotrexate or other immunosuppressive agents
- Patients recently on such medications
- Patients with pre-existing hepatic concerns
Recent evidence from DMARD-naïve patients with early rheumatoid arthritis demonstrated that the fixed-dose regimen (20 mg daily from day 1) achieved superior ACR20 response rates (77.8%) compared to the loading-dose regimen (58.5%), with fewer adverse events occurring earlier in treatment 4. This challenges the traditional approach and suggests the loading dose provides no incremental benefit in early disease.
Maintenance Dosing
Daily dosing of 20 mg is recommended for treatment 2. A dose of 25 mg daily resulted in greater incidence of side effects including alopecia, weight loss, and liver enzyme elevations; therefore, doses higher than 20 mg daily are not recommended 2.
If the 20 mg daily dose is not well tolerated, reduce to 10 mg daily 2. Alternatively, you can start with 10 mg daily and escalate to 20 mg, or vice versa, depending on the balance between tolerability and desired rapidity of efficacy 5.
Due to the prolonged half-life of the active metabolite (M1), patients should be carefully observed after dose reduction, as it may take several weeks for metabolite levels to decline 2.
Monitoring Requirements
Baseline Monitoring
- History and physical examination
- Complete blood count with differential
- Liver function tests
- Pregnancy test if indicated 1
Ongoing Monitoring
For adults: Monthly CBC with differential and liver function tests for the first 6 months, then every 6-8 weeks thereafter 1
For pediatric patients with JIA: CBC and liver function tests are conditionally recommended within the first 1-2 months of usage and every 3-4 months thereafter 6
Dose Adjustments for Hepatotoxicity
Temporarily hold leflunomide if ALT exceeds 3 times the upper limit of normal (ULN) 6. This is per package insert recommendations and represents a clinically relevant elevation requiring intervention.
In clinical practice, when hepatic enzyme elevations occur 7:
- 20% of patients: Discontinue leflunomide; enzymes normalize within 4-6 weeks
- 50% of patients: Reduce dose from 20 mg to 10 mg daily for normalization
- 30% of patients: Continue without dose reduction (for minor elevations)
Hepatic Impairment
Leflunomide is not recommended in patients with significant hepatic impairment or evidence of hepatitis B or C virus infection 2. The drug undergoes hepatic activation and elimination, and pre-existing hepatic disease increases the risk of hepatotoxicity.
Pediatric Dosing
For pediatric patients with polyarticular juvenile rheumatoid arthritis, weight-based dosing is recommended 8:
- 10 mg daily for 10-20 kg
- 15 mg daily for 20-40 kg
- 20 mg daily for >40 kg
This achieves comparable steady-state concentrations to adults while accounting for the strong correlation between body weight and volume of distribution.
Time to Efficacy Assessment
Observe patients under leflunomide monotherapy for at least 3-4 months before assessing efficacy 5. Without the drug elimination procedure, it may take up to 2 years to reach plasma M1 metabolite levels <0.02 mg/L due to individual variation in drug clearance 2.
Key Safety Considerations
- Pregnancy Category X: Absolutely contraindicated in pregnancy 1, 2
- Women of childbearing potential must use reliable contraception and have negative pregnancy test before initiation
- Drug elimination procedure required if pregnancy occurs or is planned: Cholestyramine 8 grams three times daily for 11 days, verify plasma levels <0.02 mg/L by two separate tests at least 14 days apart 2
- Co-administration with methotrexate increases hepatotoxicity risk despite no pharmacokinetic interaction 1
- Rifampin increases leflunomide levels 1
