Ural Powder Has No Role in Cholelithiasis Treatment
Ural powder should not be used for cholelithiasis (gallstones) treatment as it is a urinary alkalinizer designed for kidney stones, not gallbladder stones. This is a fundamental misunderstanding of the medication's indication.
Why This is a Critical Error
Ural powder is a urinary alkalinizing agent containing sodium and potassium bicarbonate/citrate, specifically formulated to alter urine pH for the management of urolithiasis (kidney stones), particularly uric acid stones 1, 2, 3. The medication works by raising urinary pH to increase the solubility of uric acid and cystine in urine 3.
Cholelithiasis involves stones in the gallbladder or bile ducts—an entirely different organ system with completely different pathophysiology. Urinary alkalinizers have no mechanism of action on bile composition or gallstone dissolution.
Actual Treatment for Cholelithiasis
For Gallbladder Stones
The definitive treatment for symptomatic cholelithiasis is laparoscopic cholecystectomy 4, 5.
For patients who cannot undergo surgery, ursodeoxycholic acid (UDCA) is the appropriate medical dissolution therapy:
- Dose: 8-10 mg/kg/day, preferably as a single bedtime dose 6, 7, 8
- Duration: 1-2 years typically required 8
- Success rate: Up to 75-85% for carefully selected patients with small (<10mm), radiolucent, cholesterol-rich stones and patent cystic duct 6, 9
For Common Bile Duct Stones (Choledocholithiasis)
Endoscopic retrograde cholangiopancreatography (ERCP) with sphincterotomy and stone extraction is the mainstay of therapy, with 90% success rates 10, 11.
Common Pitfall to Avoid
Do not confuse urinary stone disease with biliary stone disease simply because both involve "stones." The biochemistry, location, and treatment are fundamentally different. Urinary alkalinizers like Ural powder affect urine pH but have zero impact on bile composition or gallstone dissolution.
If a patient presents with gallstones, refer to surgery or gastroenterology within 2 weeks 5. If medical dissolution is considered, use ursodeoxycholic acid, not urinary alkalinizers 12, 13, 14.
