Rasburicase for Tumor Lysis Syndrome
For high-risk patients, administer rasburicase 0.20 mg/kg/day (or 0.15-0.20 mg/kg/day) intravenously over 30 minutes, starting at least 4 hours before chemotherapy and continuing for 3-5 days, with dosing guided by uric acid levels. 1
Dosing Recommendations
Standard Dosing
- Dose: 0.20 mg/kg/day IV (FDA-approved range: 0.15-0.20 mg/kg) 2, 1
- Route: Intravenous infusion over 30 minutes 2, 1
- Duration: 3-5 days typically, though can range from 1-7 days based on uric acid control 2
- Timing: First dose at least 4 hours before starting tumor-specific therapy 1
Alternative Dosing Considerations
While guidelines recommend weight-based dosing, emerging research suggests single fixed doses (3-6 mg) may be effective in adults 3, 4, 5. However, prioritize guideline-recommended weight-based dosing as the standard approach, particularly for high-risk patients. The FDA label supports 0.15-0.20 mg/kg dosing 6.
Dosing Adjustments
- Massive tumor lysis: May require twice-daily administration 2
- Pediatric patients <2 years: May have higher uric acid exposure and lower response rates; monitor closely 6
- No renal dose adjustment required (unlike allopurinol) 2
Administration Route and Technique
- IV infusion only over 30 minutes in 50 mL normal saline 2
- Do NOT administer concomitantly with allopurinol - causes xanthine accumulation and removes substrate for rasburicase 1
- After completing rasburicase course, transition to oral allopurinol 1
Absolute Contraindications
Screen all patients before administration:
G6PD deficiency (most critical) - can cause life-threatening hemolysis 2, 1, 6
- Higher risk in African American, Mediterranean, and Southeast Asian descent
- Obtain thorough history of drug-induced hemolytic anemia
- Perform definitive testing including RBC NADPH formation measurement 2
Known hypersensitivity/anaphylaxis to rasburicase or urate oxidase 2
History of methemoglobinemia 1
Pregnancy and lactation 6
- Animal studies show fetal harm at 5× human exposure
- Avoid breastfeeding during treatment and for 2 weeks after last dose 6
Relative Contraindications
- History of asthma or significant atopic allergy - limited experience, use with caution 2
Monitoring Parameters
Pre-Treatment Assessment
- Mandatory screening: G6PD status, creatinine clearance/eGFR, serum LDH 1
- Renal ultrasound in all patients undergoing chemotherapy 1
During High-Risk TLS Prevention
Monitor every 12 hours for first 3 days, then every 24 hours 1:
- Uric acid
- Electrolytes: potassium, phosphorus, calcium, sodium
- Renal function: creatinine, BUN
- LDH
During Active TLS Treatment
Monitor every 6 hours for first 24 hours, then daily 1:
- Vital signs (heart rate, blood pressure, urine output, respiratory rate)
- Serum uric acid
- Electrolytes (phosphate, calcium, potassium)
- Renal function (creatinine, BUN, urine pH, osmolality, specific gravity)
- Daily: CBC, LDH, albumin, serum osmolality, blood gases, ECG, body weight
Critical Monitoring Caveat
Uric acid samples must be placed immediately on ice and analyzed within 4 hours to prevent ex vivo enzymatic degradation by rasburicase, which causes falsely low readings 2, 6. This is a common pitfall that can lead to inappropriate dosing decisions.
Supportive Care Requirements
Hydration (Essential)
- Adults: Maintain urine output ≥100 mL/hour 1
- Children <10 kg: 3 mL/kg/hour 1
- Pediatric standard: 2-3 L/m²/day IV (200 mL/kg/day if <10 kg) 2
- Use loop diuretics if needed, except with obstructive uropathy or hypovolemia 1
Urine Alkalinization
Do NOT alkalinize urine in patients receiving rasburicase 1 - increases calcium phosphate precipitation and reduces xanthine solubility. This represents a shift from traditional TLS management.
Fluid Composition
- Initially withhold potassium, calcium, and phosphate from IV fluids due to TLS risks 2
- Maintain urine specific gravity at 1.010 2
Efficacy Expectations
- 97-99% of patients achieve uric acid normalization (<476 μmol/L or <8 mg/dL) within 4 hours of first dose 2, 3, 7
- 84% achieve undetectable levels (<0.7 mg/dL) 3
- Treatment duration guided by uric acid levels; discontinue when extremely low or undetectable 2
Adverse Events
Serious (Rare)
- Anaphylaxis (including fatal cases) 6
- Hemolysis (in G6PD deficiency)
- Methemoglobinemia 2, 6
- Seizures, respiratory distress, sepsis 2
Common
Clinical Pearls
High-risk patients requiring rasburicase include those with: bulky disease, WBC >50,000/mm³, LDH >2× upper normal limit, pre-existing renal impairment, hyperuricemia (>8 mg/dL children, >10 mg/dL adults), high-grade lymphomas, or acute lymphoblastic leukemia 1
Rasburicase allows earlier chemotherapy initiation compared to allopurinol due to rapid uric acid degradation 1
For patients with preexisting hyperuricemia (≥7.5 mg/dL), rasburicase is preferred over allopurinol 2
