Adalimumab Route and Injection Site
Adalimumab is administered subcutaneously, with injections given in the thigh or abdomen, rotating sites with each dose. 1
Route of Administration
Adalimumab is given via subcutaneous (SC) injection only 2. This is consistent across all FDA-approved indications including psoriasis, rheumatoid arthritis, inflammatory bowel disease, and hidradenitis suppurativa.
Specific Injection Sites
The recommended injection sites are:
- Thigh
- Abdomen
Key Administration Guidelines
Site rotation is mandatory - injections must occur at separate sites with each administration. Avoid injecting into areas where the skin is:
- Tender
- Bruised
- Red
- Hard
Delivery Methods
Adalimumab can be self-administered using three delivery systems, all providing the same subcutaneous route 1:
- Auto-injector (AI) - available in 40 mg/0.4 mL and 80 mg/0.8 mL doses
- Prefilled syringe with safety guard - available in 40 mg/0.4 mL and 80 mg/0.8 mL doses
- Standard prefilled syringe - available in 20 mg/0.2 mL, 40 mg/0.4 mL, and 80 mg/0.8 mL doses
Patient Preference Data
Research demonstrates that patients significantly prefer the auto-injector pen over prefilled syringes 3. In a clinical trial, 88.5% of patients preferred the pen device, reporting it as less painful, easier to use, more convenient, and requiring less time to inject 3. The injection site pain scores were equivalent between devices, but patient perception favored the pen 4, 3.
Common Pitfalls to Avoid
- Never administer intravenously - adalimumab is exclusively for subcutaneous use
- Do not inject into the same site repeatedly - this increases risk of injection site reactions, the most common adverse effect 5
- Allow medication to reach room temperature (15-30 minutes) before injection to reduce injection site pain 1
- Do not shake the medication - this can denature the antibody 1
