How to Calculate Metabolic Tumor Volume (MTV)
MTV is calculated by performing 3D segmentation of FDG-avid lesions using isocontour thresholds, with the EANM recommending either 41% or 50% of the maximum pixel value (SUVmax) as standardized approaches 1.
Recommended Calculation Methods
The European Association of Nuclear Medicine (EANM) guidelines provide the authoritative framework for MTV calculation 1:
Standard Threshold-Based Approach
Use 3D isocontour segmentation with these specific thresholds:
- MTV41: Volume derived using 41% of SUVmax threshold (VOI41)
- MTV50: Volume derived using 50% of SUVmax threshold (VOI50)
The MTV represents the total volume encompassed by the chosen isocontour threshold 1.
Step-by-Step Calculation Process
Identify the lesion on original reconstructed PET images (no resampling, smoothing, or manipulation allowed) 1
Determine SUVmax - the single voxel with highest uptake in the lesion 1
Apply the threshold:
- For MTV41: Create 3D isocontour at 41% of SUVmax
- For MTV50: Create 3D isocontour at 50% of SUVmax
Measure the volume enclosed by the isocontour - this is your MTV 1
Visually verify the segmentation corresponds to the actual lesion 1
Choosing Between MTV41 and MTV50
Use MTV41 as your first choice - it corresponds best with actual tumor dimensions, but only when you have:
- High tumor-to-background ratios
- Homogeneous tracer uptake 1
Switch to MTV50 when you encounter:
- High background activity
- Adjacent high-uptake structures (bladder, heart)
- Low lesion uptake
- Heterogeneous tracer distribution 1
Critical Pitfall to Avoid
Once you select a threshold method (MTV41 or MTV50), you must use the same method for all subsequent scans in that patient - this is essential for longitudinal assessment and treatment response monitoring 1.
Alternative Threshold: MTV2.5
While not in the primary EANM recommendations, an absolute SUV threshold of 2.5 (MTV2.5) shows excellent reproducibility with an intraclass correlation coefficient of 0.97 and correlates well with anatomical tumor volume 2. This fixed threshold approach may be simpler than percentage-based methods and demonstrates comparable predictive performance for aggressive disease features 2.
Quality Control Requirements
Always visually inspect semiautomatically generated volumes 1. The segmentation may fail or be unreliable when:
- High background activity is present
- High-uptake structures are adjacent to the lesion
- Tracer uptake is heterogeneous (may only capture the most metabolically active portion)
If the VOI doesn't correspond visually with the lesion, abandon MTV calculation and report only SUVmax, SULmax, and possibly SUVpeak instead 1.
For Multiple Lesions (Total MTV)
When calculating total metabolic tumor volume (TMTV) across multiple lesions, apply the same threshold approach to all lesions and sum the individual MTVs. Semi-automated workflows can be implemented with nuclear medicine technologists performing initial segmentation followed by physician validation 3.
Related Metric: Total Lesion Glycolysis (TLG)
Once you have MTV, you can calculate TLG as: TLG = SUVmean × MTV 1. Use the same VOI approach (MTV41 yields TLG41, MTV50 yields TLG50).
