Contraindications of Brimonidine Eye Drops

Brimonidine tartrate ophthalmic solution is absolutely contraindicated in patients with hypersensitivity to brimonidine or any component, those receiving MAO inhibitor therapy, neonates, infants, young children (especially under age 2), and breastfeeding mothers. 1

Absolute Contraindications

FDA-Labeled Contraindications

  • Hypersensitivity to brimonidine tartrate or any medication component 1
  • Concurrent MAO inhibitor therapy 1

Age-Related Contraindications (Critical Safety Issue)

  • Neonates and infants - Risk of life-threatening CNS depression including apnea, bradycardia, hypotension, hypothermia, hypotonia, and somnolence 1, 2, 3
  • Young children under 2 years - Multiple case reports document severe toxicity including cardiopulmonary resuscitation requirements 4, 3
  • Children under 6-7 years should be used with extreme caution - Dose-response CNS depression documented even with therapeutic dosing 2, 5

Breastfeeding

  • Absolutely contraindicated in breastfeeding mothers - Brimonidine crosses the blood-brain barrier and can cause apnea and somnolence in nursing infants 6, 7

Critical Safety Considerations

Why Children Are at Extreme Risk

The pediatric population faces disproportionate danger because:

  • Ocular dosing is not weight-adjusted, leading to relative overdosing in small children 8
  • Immature blood-brain barrier allows greater CNS penetration 8
  • Inefficient drug metabolism in infants 8
  • Cases have required cardiopulmonary resuscitation 3

Clinical Presentation of Toxicity

When poisoning occurs, expect:

  • Lowered consciousness/unresponsiveness 2, 4, 3
  • Hypotonia 1, 3
  • Apnea (may be naloxone-resistant) 3
  • Bradycardia and hypotension 1, 3
  • Pupils may be unremarkable (absence of miosis does not rule out toxicity) 3

Relative Contraindications/Use With Caution

Cardiovascular Disease

  • Severe cardiovascular disease requires caution, though not an absolute contraindication like with beta-blockers 9
  • Monitor for blood pressure and heart rate changes 9

Elderly Patients

  • Increased risk of CNS depression 7

Pregnancy

  • Pregnancy Category B rating 6
  • May be considered prior to last trimester when benefits outweigh risks 7
  • Beta-blockers generally preferred during pregnancy due to longer safety experience 6
