Plasma Copeptin Measurement: Clinical Indications and Reference Ranges
Plasma copeptin should be measured primarily for early rule-out of myocardial infarction when high-sensitivity troponin assays are unavailable, and for differential diagnosis of polyuria-polydipsia syndromes, with normal reference ranges of approximately 1-13.8 pmol/L in healthy adults (higher in men than women). 1, 2, 3
Primary Clinical Indications
Acute Coronary Syndrome (ACS) Evaluation
Copeptin is recommended as an additional biomarker for early rule-out of MI specifically when sensitive or high-sensitivity cardiac troponin assays are not available. 1 The 2015 ESC guidelines explicitly state that copeptin quantifies endogenous stress levels at MI onset, and when combined with conventional (less sensitive) troponin assays, provides substantial added diagnostic value.
- Dual-marker strategy: Normal cardiac troponin combined with copeptin <10 pmol/L at presentation shows very high negative predictive value for MI, potentially eliminating the need for serial testing in selected patients 1
- Copeptin may have some added value even over high-sensitivity troponin for early MI rule-out, though this is less established 1
- This application is most relevant in settings without access to high-sensitivity troponin assays
Polyuria-Polydipsia Syndrome Differential Diagnosis
Baseline plasma copeptin levels above 21.4 pmol/L are diagnostic for nephrogenic diabetes insipidus (NDI) in adults. 2 This represents the most established diagnostic application of copeptin measurement.
- Copeptin serves as a stable surrogate for arginine vasopressin (AVP), which is unstable and difficult to measure reliably 2, 3, 4
- Values significantly higher than 21.4 pmol/L distinguish NDI from other polyuric disorders with high specificity 2
- For adults with copeptin <21.4 pmol/L, additional testing with hypertonic saline or arginine infusion is needed to differentiate AVP deficiency (central DI) from primary polydipsia 2
- Limited pediatric data exists for diagnostic thresholds 2
Normal Reference Ranges
Healthy Adults
- General population: 1-13.8 pmol/L (median 4.2 pmol/L) 3
- Sex differences: Significantly higher in men than women 3, 5
- Copeptin is detectable in 97.5% of healthy individuals 3
Pregnancy-Specific Ranges
- 30th gestational week: Median 3.377 pmol/L (reference range 1.343-7.829 pmol/L) 6
- 36th gestational week: Median 4.735 pmol/L (reference range 2.06-13.2 pmol/L) 6
- Values can exceed 10 pmol/L after 36 weeks, potentially indicating cardiovascular/endothelial overload 6
Important Clinical Considerations
Physiological Variations
- Circadian rhythm: Minor circadian variation exists, with trend toward higher levels (5.9 pmol/L) at night/early morning (4-6 AM) and lowest levels (2.3 pmol/L) in late afternoon (5-7 PM), though this pattern is only observed in individuals with higher basal copeptin levels 7
- Fasting status: Significantly influenced by fasting and water load 3
- Exercise: Increases significantly after physical activity 3
- Time of day: Can generally be measured irrespective of time due to minor circadian variation 7
Specimen Stability
A major advantage of copeptin over AVP is its exceptional stability:
- Stable for at least 7 days at room temperature 3
- Stable for 14 days at 4°C 3
- Stable in serum, EDTA plasma, heparin plasma, and citrate plasma 3
- Results available within 3 hours 3
Biological Variation
- Within-subject biological variation (CVI): 18-19% 5
- Reference change values: -35% (decreasing) to +54% (increasing) 5
- Marked individuality observed 5
- Critical caveat: BV estimates only apply to subjects who abstained from oral intake for at least 8 hours, as fluids have rapid and marked effects on copeptin physiology 5
Common Pitfalls to Avoid
Do not use copeptin for ACS diagnosis if high-sensitivity troponin is available - the guidelines are clear that copeptin's role is primarily when sensitive assays are unavailable 1
Do not interpret copeptin in isolation - always integrate with detailed clinical assessment, ECG findings, and other biomarkers 1
Do not measure copeptin without ensuring proper fasting status - at least 8 hours of food and fluid abstinence is required for reliable interpretation 5
Do not apply adult diagnostic thresholds to pediatric populations - limited pediatric data exists and age-specific values are needed 2
Do not forget sex-specific differences - men have significantly higher baseline values than women 3, 5
In pregnancy, do not use non-pregnant reference ranges - gestational age-specific ranges are required, particularly after 30 weeks 6
