What is Dotti and is it progesterone-only?
Dotti is NOT progesterone-only—it is an estradiol-only transdermal patch that contains no progesterone whatsoever. 1
What Dotti Contains
Dotti (estradiol transdermal system) is a prescription patch that delivers only estradiol (an estrogen hormone) through the skin into the bloodstream 1. It comes in five different strengths that deliver 0.025,0.0375,0.05,0.075, or 0.1 mg of estradiol per day 1.
Critical Safety Consideration: Progestogen Requirement
If you have a uterus, you MUST add a progestogen to Dotti to prevent endometrial hyperplasia and cancer 2. Using estrogen alone in women with an intact uterus significantly increases the risk of uterine cancer 1. The FDA label explicitly warns that "using estrogen-alone increases your chance of getting cancer of the uterus (womb)" 1.
When Progestogen is Required:
- Women with a uterus: Must receive concomitant progestogen therapy 2
- Women who have had a hysterectomy: Can use estradiol alone without progestogen 2
Progestogen Regimen Options:
The Endocrine Society guidelines 2 specify two approaches:
- Sequential regimen: Progestogen for 12-15 days each month (preferred for recently menopausal women prone to breakthrough bleeding)
- Continuous regimen: Daily progestogen on an ongoing basis
What Dotti is Used For
Dotti is FDA-approved for 1:
- Reducing moderate to severe hot flashes
- Treating moderate to severe vaginal/vulvar atrophy (menopausal changes in and around the vagina)
- Treating hypoestrogenism in women whose ovaries don't produce enough estrogen naturally
- Preventing osteoporosis in postmenopausal women
Why Transdermal Estradiol Matters
Transdermal estradiol patches like Dotti offer advantages over oral estrogen 2:
- Avoid first-pass hepatic metabolism, reducing procoagulant effects
- Lower risk of blood clots (VTE) compared to oral estrogen
- Preferable for women with: hypertension, hypertriglyceridemia, obesity, metabolic syndrome, diabetes, history of gallbladder disease, or VTE risk 2
The transdermal route delivers physiological estradiol levels without the dramatic increases in estrone and estrone sulfate seen with oral preparations 2.
Common Pitfall to Avoid
Do not confuse estrogen-only therapy with progesterone-only therapy. Dotti contains zero progesterone. Women often need both hormones prescribed separately—Dotti for estrogen replacement, plus an additional progestogen medication if they have a uterus. Failing to add progestogen in women with an intact uterus is a serious prescribing error that substantially increases endometrial cancer risk 2, 1.
