Atomoxetine and Clonazepam Co-Administration
Atomoxetine and clonazepam can be safely prescribed together without dose adjustments, as there are no clinically significant pharmacokinetic interactions between these medications.
Dosing Guidelines for Atomoxetine
Children and Adolescents (≤70 kg)
- Initial dose: 0.5 mg/kg/day 1
- Target dose: 1.2 mg/kg/day after minimum 3 days 1
- Maximum dose: 1.4 mg/kg/day or 100 mg/day, whichever is less 1
- Administration: Single morning dose OR divided doses (morning and late afternoon/evening) 1
Children and Adolescents (>70 kg) and Adults
- Initial dose: 40 mg/day 1
- Target dose: 80 mg/day after minimum 3 days 1
- Maximum dose: 100 mg/day after 2-4 additional weeks if needed 1
Drug Interaction Considerations
No interaction exists between atomoxetine and clonazepam. The evidence is clear on this point:
- Atomoxetine is metabolized primarily through CYP2D6 2, 3
- Clonazepam does not inhibit or induce CYP2D6
- In vitro studies showed atomoxetine does not significantly inhibit CYP1A2, CYP2C9, CYP2D6, or CYP3A at therapeutic doses 3
- Clinical studies confirmed no clinically significant interactions with CYP2D6 or CYP3A substrates 3
Important CYP2D6 Considerations
Dose adjustment IS required when atomoxetine is combined with strong CYP2D6 inhibitors (paroxetine, fluoxetine, quinidine) or in CYP2D6 poor metabolizers 1:
- Children ≤70 kg: Start 0.5 mg/kg/day, increase to 1.2 mg/kg/day only after 4 weeks if needed
- Children >70 kg and adults: Start 40 mg/day, increase to 80 mg/day only after 4 weeks if needed
Clinical Considerations When Combining These Medications
Complementary Mechanisms
- Atomoxetine: Norepinephrine reuptake inhibitor, increases norepinephrine and dopamine in prefrontal cortex 2
- Clonazepam: Benzodiazepine, enhances GABA activity
- These mechanisms do not overlap or interfere with each other
Monitoring Parameters
When prescribing both medications together, monitor:
- Heart rate and blood pressure (modest increases expected)
- Suicidal ideation, especially first few months or dose changes (FDA black box warning)
- Gastrointestinal symptoms (nausea, vomiting, abdominal pain)
- Appetite and growth parameters
- Hepatic function (rare hepatitis risk)
For Clonazepam 4:
- Sedation/somnolence (most common side effect)
- Blood pressure and heart rate (may decrease)
- Avoid abrupt discontinuation (taper to prevent rebound hypertension)
Onset of Action
- Atomoxetine: 6-12 weeks for full effect 2
- Clonazepam: Immediate anxiolytic effects
This delayed onset for atomoxetine means clonazepam may provide symptom relief while waiting for atomoxetine to reach therapeutic effect.
Special Populations
Hepatic Impairment
Atomoxetine requires dose reduction 1:
- Moderate (Child-Pugh B): Reduce to 50% of normal dose
- Severe (Child-Pugh C): Reduce to 25% of normal dose
Comorbid Anxiety
One case report demonstrated successful use of atomoxetine augmentation with clonazepam in treatment-resistant panic disorder 5, suggesting this combination may be particularly useful when ADHD coexists with anxiety disorders.
Common Pitfalls to Avoid
- Don't titrate atomoxetine too rapidly - increases gastrointestinal side effects 4
- Don't expect immediate results - atomoxetine requires 6-12 weeks for full therapeutic effect 2
- Don't abruptly discontinue clonazepam - taper to avoid rebound hypertension 4
- Don't forget cardiovascular screening - obtain personal and family cardiac history before starting atomoxetine 4
- Don't overlook CYP2D6 interactions - check for concurrent SSRIs that inhibit CYP2D6 1
