Maximum Dose for Symbicort
The maximum recommended dose of Symbicort is 2 inhalations of the 160/4.5 mcg strength twice daily (total daily dose: 640 mcg budesonide/18 mcg formoterol) for adults and adolescents ≥12 years with asthma or COPD. 1
Dosing by Age and Indication
Adults and Adolescents (≥12 years)
- Maximum dose: Symbicort 160/4.5 mcg, 2 inhalations twice daily
- Total daily maximum: 640 mcg budesonide/18 mcg formoterol
- This applies to both asthma and COPD treatment 1
Children (6 to <12 years)
- Only approved strength: Symbicort 80/4.5 mcg
- Dose: 2 inhalations twice daily
- Total daily maximum: 320 mcg budesonide/9 mcg formoterol
- The 160/4.5 mcg strength is NOT approved for this age group 1
Children <6 years
- Symbicort is not indicated for children under 6 years of age 1
Critical Dosing Warnings
More frequent administration or higher number of inhalations than 2 inhalations twice daily is NOT recommended, as patients are more likely to experience adverse effects with higher doses of formoterol 1. This is an explicit FDA warning that should not be exceeded.
Special Consideration: SMART/MART Regimen
When using Symbicort as Single Maintenance and Reliever Therapy (SMART), a different maximum applies:
- Maintenance dose: 1-2 inhalations of 160/4.5 mcg once or twice daily
- Plus as-needed: Additional inhalations of 160/4.5 mcg for symptom relief
- Absolute maximum in any single day: 12 total inhalations (delivering 54 mcg formoterol total) 2
This SMART approach is supported by the 2020 NAEPP guidelines for steps 3-4 asthma management 3 and allows flexible dosing driven by symptoms, but the 12-inhalation daily ceiling must never be exceeded.
Key Clinical Pitfalls
- Do not combine with other LABAs: Patients using Symbicort should not use additional long-acting beta-agonists for any reason 1
- COPD-specific: Only the 160/4.5 mcg strength is indicated for COPD; the 80/4.5 mcg strength is not approved for this indication 1
- Stepping down: If asthma is well-controlled for ≥3 consecutive months, consider stepping down the dose rather than maintaining maximum dosing 3
