Forteo Dosing for Osteoporosis
The recommended dose of teriparatide (Forteo) is 20 mcg administered subcutaneously once daily into the thigh or abdominal region. 1
Administration Details
- Route: Subcutaneous injection only (not approved for intravenous or intramuscular use) 1
- Injection sites: Thigh or abdominal region 1
- Frequency: Once daily 1
- Initial administration: Should be given under circumstances where the patient can sit or lie down if orthostatic hypotension occurs 1
Treatment Duration
Treatment should be limited to a maximum of 2 years during a patient's lifetime unless the patient remains at or returns to very high fracture risk. 1 This limitation stems from the osteosarcoma risk observed in rat studies, though importantly, no increased osteosarcoma incidence has been documented in over 1 million human users since 2002. 2
Supplementation Requirements
Patients should receive supplemental calcium and vitamin D if daily dietary intake is inadequate. 1 This ensures optimal bone formation response to the anabolic therapy.
Patient Selection
Teriparatide is specifically indicated for:
- Postmenopausal women with osteoporosis at very high risk of fracture (severe or multiple vertebral fractures) 2
- Men with primary or hypogonadal osteoporosis at high fracture risk 1
- Patients with glucocorticoid-induced osteoporosis (≥5 mg prednisone daily equivalent) 1
- Those who have failed or are intolerant to other osteoporosis therapies 3, 1
Critical Sequential Therapy Requirement
Following completion of teriparatide therapy, patients must transition to antiresorptive therapy (bisphosphonates, denosumab, or other agents) to maintain bone density gains. 2 This is essential because the anabolic effects of teriparatide rapidly reverse after discontinuation, with most benefits lost within 1 year of stopping treatment. 2
Practical Delivery Considerations
- The prefilled delivery device (pen) contains 560 mcg/2.24 mL (250 mcg/mL) 1
- Each pen delivers 28 daily doses of 20 mcg 1
- Discard the pen 28 days after first use 1
- Patients require proper training on pen use from qualified healthcare professionals 1
Important Contraindications and Precautions
Avoid teriparatide in patients with:
- Open epiphyses (pediatric/young adults) 1
- Paget's disease or other metabolic bone diseases 1
- Prior skeletal radiation therapy 1
- Pre-existing hypercalcemia or primary hyperparathyroidism 1
- Active urolithiasis (use with caution) 1
Monitor serum calcium before initiating therapy, as teriparatide can cause hypercalcemia. 2, 1
Common Adverse Effects
The most frequent side effects (>10%) include arthralgia, pain, and nausea. 1 Other notable effects include dizziness, leg cramps, vomiting, headache, and palpitations, which may lead to treatment discontinuation. 3, 2 These are generally mild and transient.
